Job Summary

We are seeking a dedicated and experienced Home Healthcare RN Clinical Manager to lead our home health nursing operations. In this vital leadership role, you will oversee clinical services, coordinate patient care, and manage nursing staff to ensure the delivery of high-quality, patient-centered care in a home setting. The ideal candidate will possess extensive clinical expertise, including critical care, acute care, and long-term care experience, combined with strong management skills. Your leadership will support compliance with healthcare regulations, optimize patient outcomes, and foster a collaborative team environment. Key Responsibilities:

Supervise and coordinate the daily activities of nursing staff providing home healthcare services
Develop and implement clinical protocols aligned with industry standards such as NCQA and HIPAA regulations
Oversee patient assessments, care planning, and discharge planning to ensure comprehensive and personalized care
Monitor compliance with EMR/EHR systems like Epic, Cerner, or Athenahealth for accurate medical documentation
Facilitate ongoing staff training on topics including IV infusion, catheterization, sterile technique, and infection control
Conduct utilization review and management to optimize resource use while maintaining quality standards
Collaborate with interdisciplinary teams to support patients with complex needs such as dementia care, behavioral health, or developmental disabilities
Ensure adherence to safety protocols including airway management, ventilator care, and sterile processing
Manage staffing schedules, supervise nurse management activities, and promote professional development
Support quality improvement initiatives by analyzing patient outcomes and implementing best practices

Experience

Proven clinical expertise in acute care environments such as ICU or Level I Trauma Centers
Extensive experience working with diverse patient populations including geriatrics, pediatrics (including neonatal ventilator care), and individuals with disabilities
Demonstrated leadership in supervising nursing teams within home healthcare or related settings
Proficiency in medical terminology, ICD-9/10 coding, CPT coding, and medical documentation standards
Familiarity with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks
Knowledge of utilization review processes and case management strategies
Experience managing patients requiring complex interventions such as tube feeding, medication administration via injection or venipuncture, catheter care, and physical examinations
Background in managing patients with chronic conditions like dementia or those requiring long-term ventilator support
Strong understanding of healthcare regulations including HIPAA compliance and NCQA standards
Prior experience working in hospital medicine settings or emergency medicine is advantageous
Supervisory experience overseeing nursing staff in home or institutional settings is preferred
