Full Job Description

Interdisciplinary, IDT, Data Coordinator

Department(s): Clinic Support

Reports to: Center Director

FLSA status: Non-Exempt

EEOC Classification: Administrative

Job Summary

Responsible for the day-to-day support of all Interdisciplinary Team (IDT) functions and coordination/support of the IDT meetings and services. Collaborate with Clinical Services Manager and Medical Director for tracking service delivery and regulatory compliance.

Position Responsibilities

Assists in the preparation and facilitation of the IDT, supporting it to achieve its goals, and ensuring ongoing operational efficiency by preparing the daily agenda, taking minutes, and daily tracking of ER visits and Hospitalizations. Monitors IDT approved deadlines, goals, and timelines.
Assists Center Director in the coordination of PACE program activities including assessments, care planning, preparation for IDT conferences, daily morning briefings, service delivery requests, and all related follow-up, tracking, and documentation.
Monitors and informs individual IDT members of gaps and timelines related to SDR, care plans and provision of services (POS) needs, assuring timely completion.
Assists Center Director with administrative support, including data entry, providing reports, scheduling meetings, and taking minutes for committee meetings, facilitating mailers and other communications, and any other reporting or data entry assigned.
Assists Center Director and Clinical Services Manager with tracking of staff required trainings and in-services, including providing support with scheduling.
Assists Center Director and Clinical Services Manager with preparation and participation in Centers for Medicare & Medicaid Services (CMS) and Department of Health Care Services (DHCS) audits and subsequent corrective action plans.
Knowledgeable with CMS PACE regulations relating to Service Determination Requests, Care Plans, and Provision of Services and serves as subject matter expert for the team
Ensures timely patient correspondence and notification of IDT decisions related to SDRs, involuntary disenrollments, letters to schedule appointments, annual updates to enrollment agreement, etc.
Participates in and supports ongoing Performance Improvement (PI) and LEAN activities.
Participates in the program’s policy and procedure development.
Learns Electronic Health Record (HER) system and serves as a superuser to assist others, troubleshoot issues, etc.
Other duties as assigned.

Possess the Ability To

Ability to work independently. Strong administrative and organizational skills. Ability to train and delegate responsibilities within an Interdisciplinary Team (IDT).
Ability to comply with program requirements and applicable government regulations, including procedures for documenting patient information, including HIPPA requirements.
Sound verbal and written communication skills to convey information effectively. Ability to communicate effectively and openly with other team members in a manner reflecting the AgeWell PACE values.
Excellent interpersonal skills that result in maintaining a positive attitude, team building and contribution to effective work relationships and successful negotiation with coworkers, management, participants, and community contacts.
Understands the medical aspects of clinical operations and terminology used in the clinical setting.
Ability to use good judgment, resourcefulness, flexibility, and problem-solving skills.
Ability to prioritize and complete duties within an agreed upon time frame and adjust personal schedule as required and adapt appropriately to changes in priorities and workload.
Strong basic computer skills (Microsoft Outlook, Word, Power Point, and Excel). Ability to navigate, use, and maintain database software programs, and familiarity with industry and company applications and databases that contain medical, patient, and healthcare information (Preferred EHR systems are PACELOGIC, Epic, etc.).
Flexibility, initiative and willingness to participate in all aspects of the organization.
Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
Pass a criminal background which will verify that the candidate is: not excluded from participation in Medicare and Medicaid programs; has not been convicted of criminal offenses related to their involvement in Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act; and have not been convicted of physical, sexual, drug or alcohol abuse.

Experience & Education

Bachelor’s Degree preferably in healthcare or business management or equivalent experience preferred, but not required
Certification in Information Technology, Cloud Computing & Infrastructure, etc.
Exercise sound judgement and exhibit strong decision-making and problem-solving skills
Detail oriented, with strong multi-tasking skills
Excellent writing, proofreading, and editing skills.
Proficient with software including Microsoft Word, Excel, and Outlook.
Experience working in a nonprofit, strongly preferred.
Experience working in a healthcare setting, preferred.

Physical demands: Employee must be able to sit for extended periods of time. Employee will also be actively present throughout the PACE center, engaged with employees, participants and with staff. Employee is required to use hands and fingers, especially for typing on the computer and using the mouse. Employee must be able to talk and hear, particularly for daily interactions with staff and elderly participants.
Work Environment: Employee must be able to work in a busy and demanding clinic environment with moderate to loud noise levels.
