Summary
Under the supervision of license staff members, assist in providing patient care in a clinical environment, utilize appropriate equipment and resources, prepare records, and maintain a clean and safe environment for patients.
Responsibilities
1. Care for clients neonate 1yr, Child/Pediatric 1-11yrs, Adolescent 12-17yrs, Adult 18-55yrs and geriatric >55yrs throughout the facility while communicating positively and respectfully.
2. Perform required tasks without prompting and adjust appropriately to situations.
3. Adhere to the attendance policy.
4. Follow district and department policies.
5. Follows Infection Control Policies and Procedures.
6. Attend required meetings and in-services.
7. Participate in Hospital District Safety Program and QA/PI Programs.
8. Use time and resources wisely.
9. Maintain HIPAA, keeping patient and hospital confidentiality.
10. Maintain adequate supplies of items needed to perform job; order or report low items to supervisor.
11. Practice proper phone etiquette, identification of self and department.
12. Report adverse events regarding patients, guests, or self to supervisor immediately.
Essential Functions
Primary Responsibilities for all Units Assigned:
Assures that each patient receives quality care.
1. Performs personal care according to individual needs including bathing, perineal care, shaving, grooming hair, mouth/ oral care as condition warrants and as directed, feeding patient, and collecting specimens.
2. Always keeps the patient’s room neat and tidy, patient dry and free of secretions and answers call lights promptly.
3. Performs procedures designed to prevent physical deterioration of skin to include patient repositioning, giving skin care, getting patients out of bed, ambulating as directed, working gently and handling patients with special conditions in a specified manner and performing catheter care as directed.
4. Provides age-appropriate care.
Provides clinical services and care for patients.
1. Takes and records vital signs, Glucometer readings, weight, and length measurements.
2. Assists physicians and licensed staff with preparing patients for various types of examinations and procedures.
3. Applies or changes dressings as requested.
4. Performs basic life support skills in an emergency.
5. Follows aseptic procedures and universal precautions when involved in direct patient contact or handling potentially infectious materials.
6. Assists physician and licensed staff with patient care.
Provides respect to patients, family, and co-workers.
1. Avoids unnecessary exposure during care.
2. Speaks politely to and considers preferences of patients.
3. Includes patients in decision making.
4. Introduces self and states explain why they are there.
5. Refrains from engaging in casual conversations with coworkers in open areas in view of patients and visitors.
6. Performs proper body substance isolation when handling potentially infectious materials or direct contact.
Participates in patient care management.
1. Reports important facts or observations to LVN, RN, or supervisor.
2. Attends shift report and makes notes of facts that are important to nursing care.
3. Assists in the interfacility transportation of patients.
Assists with maintaining patient’s files.
1. Prior to examination, obtains subjective information related to patients' perception of health.
2. Collects necessary information received concerning x-ray and laboratory reports, data from referring physicians, and surgical or other procedure reports for patient's files.
Maintains supplies and equipment.
1. Ensures that inventory of necessary supplies and equipment is maintained for diagnostic and treatment procedures.
2. Is responsible for stocking supplies in patients’ rooms and assures the cleanliness of the rooms.
3. Cleans and prepares surgical instruments for sterilization.
Functions proficiently in unit modalities across all nursing units.
Maintains accurate and timely records according to hospital policy and procedures.
1. Documents vital signs, intake and output, and appropriate portion of admission history as directed.
Other Duties as assigned.
Requirements:
Professional Education Requirements
1. High School Graduate or Equivalent.
2. Certified Nursing Aide preferred.
3. BLS
4. Nova Strip Glucose Monitor Education Certificate
Skill Requirements
1. Effective oral and written communication skills
2. Proficient in note taking/documentation.
