Full Job Description

Make a Direct Impact in the Lives of Young Adults

California Impact Coalition Foundation is seeking a motivated, compassionate, organized, and dependable Case Manager to join our Transitional Housing Program for Non-Minor Dependents (THP-NMD).

Our program serves young adults ages 18–21 who are transitioning from foster care toward independent adulthood. This is a hands-on position for someone who wants to do meaningful work while helping young adults develop the skills, stability, and confidence necessary to successfully live independently.

This is not solely a desk-based case management position. Our Case Managers work directly with participants in their homes and communities, maintain consistent communication, coordinate with county and community partners, and help participants navigate the real-life challenges of becoming independent adults.

We are looking for someone who can build positive professional relationships with young adults while maintaining clear boundaries, accountability, consistency, and strong documentation.

What You'll Do

As a THP-NMD Case Manager, you will manage an assigned caseload and serve as a primary support person for program participants.

Responsibilities include:

Meet with assigned participants in person at least once per week
Maintain regular contact through phone, text, home visits, and community meetings
Build professional relationships that promote accountability, growth, and self-sufficiency
Monitor participants' progress toward their independent living goals
Review participants' Transitional Independent Living Plans (TILP)
Develop and maintain Needs and Services Plans (NSP)
Develop and maintain Shared Living Agreements (SLA) when applicable
Conduct routine apartment/home visits and monitor health and safety
Help participants understand housing expectations and responsibilities
Identify concerns and develop corrective action plans when necessary
Coordinate services with county social workers, probation officers, eligibility workers, behavioral health providers, medical providers, schools, employers, housing providers, and community agencies
Respond appropriately to participant crises and assist with de-escalation and accessing emergency resources
Participate in the agency's on-call system as assigned
Attend staff meetings, supervision, and required trainings

Help Young Adults Build Real-World Skills

A major part of this position is helping participants develop the skills necessary to successfully transition into adulthood.

You may provide coaching and support with:

Budgeting and financial literacy
Employment readiness and job retention
Education and vocational planning
Household and apartment management
Meal planning and cooking
Transportation
Time management
Conflict resolution
Healthy relationships
Accessing community resources
Parenting skills when applicable

The goal is not to do everything for participants. The goal is to help participants develop the skills and confidence to increasingly do these things for themselves.

Documentation & Compliance

Strong documentation skills are essential for this position.

The Case Manager is responsible for maintaining accurate and timely:

Weekly case notes
Monthly progress reports
Needs and Services Plans
Shared Living Agreements
Service referrals
Participant contact records
Home-visit documentation
Incident reports
County-required documentation

Case Managers are expected to maintain organized and compliant participant files and communicate significant concerns promptly to program leadership.

Transportation Requirements

Because participants live throughout our service area, this position requires regular local travel.

Candidates must maintain:

A valid California driver's license
Current automobile insurance
Reliable transportation
Ability to travel throughout assigned service areas

Mileage reimbursement is provided for approved work-related travel.

Compensation

$25.00–$30.00 per hour, depending on experience and qualifications.
