RN Case Manager Responsibilities
· Perform comprehensive patient assessments and develop individualized care plans
· Provide skilled nursing visits in accordance with physician orders
· Monitor patient progress and evaluate physical, psychosocial, environmental, and caregiver needs
· Coordinate care with physicians, therapists, social workers, and other healthcare professionals
· Communicate changes in patient condition and recommend appropriate interventions
· Educate patients and caregivers regarding treatment plans and disease management
· Maintain accurate and timely clinical documentation
· Ensure compliance with regulatory standards and patient confidentiality requirements
Qualifications
· Current, active, unencumbered Registered Nurse (RN) license
· Minimum of one year of nursing experience
· Valid driver's license, reliable transportation, and auto insurance
· Ability to travel throughout the assigned territory
· Home Health experience preferred
· Strong communication, organization, and case management skills
Compensation & Benefits
· Salary range of $85,000 - $98,000 based on experience + over time opportunities
