Full Job Description

Job Summary
We are seeking a compassionate and dedicated Direct Care Worker to provide essential support and care to individuals with diverse needs. In this vital role, you will assist residents with daily activities, promote independence, and ensure a safe, respectful, and engaging environment. Your energy and positive attitude will help enhance the quality of life for those in your care, fostering a nurturing atmosphere where everyone feels valued and supported. This position offers an opportunity to make a meaningful difference in people's lives while working in a dynamic, team-oriented setting.

Responsibilities

Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
Monitor and record vital signs such as blood pressure, pulse, and temperature to track health status
Administer medications accurately following prescribed schedules and protocols
Support residents with memory care needs, including Alzheimer’s and dementia care strategies
Operate equipment such as Hoyer lifts safely to transfer residents with mobility challenges
Prepare nutritious meals tailored to individual dietary requirements and assist with feeding when necessary
Maintain cleanliness of resident rooms and common areas to promote hygiene and comfort
Document care activities using Electronic Medical Record (EMR) systems accurately and promptly
Engage residents in social activities to promote mental stimulation and emotional well-being
Collaborate with healthcare professionals, families, and team members to develop personalized care plans

Requirements

Prior experience in assisted living facilities, nursing homes, or home & community care settings preferred
Knowledge of ADL assistance, medication administration, vital signs monitoring, and dementia or Alzheimer’s care
Certifications such as First Aid and CPR are highly desirable
Experience working with individuals with developmental disabilities or disabilities is a plus
Ability to operate EMR systems efficiently and accurately record care activities
Valid driver’s license for driving responsibilities related to resident outings or appointments
Strong communication skills with a caring attitude focused on resident safety and dignity
Flexibility to work various shifts including weekends or evenings as needed
