Job Title: Home Health Intake & Administrative Coordinator

Job Type: Full-Time
Location: Canoga Park, CA 91303

About Us
We are a home health agency dedicated to providing quality, compassionate care to patients in the home setting. We are seeking a dependable and organized Home Health Intake & Administrative Coordinator to support our intake department and assist with general office operations.

Position Summary
The Home Health Intake & Administrative Coordinator is responsible for assisting with new patient referrals, coordinating required documentation, communicating with referral sources, and supporting the admission process. This position will also assist with general administrative duties in the office, including answering phones, filing, scanning, data entry, assisting staff, and supporting day-to-day office needs.

This role does not require a nursing license. The ideal candidate should be organized, professional, detail-oriented, and able to multitask in a busy healthcare office environment.

Responsibilities

Receive and process incoming home health referrals from hospitals, physicians, case managers, discharge planners, and other referral sources.
Collect patient demographics, insurance information, physician information, and required referral documents.
Verify insurance eligibility and assist with authorization follow-up when needed.
Request and follow up on required admission documents, including face-to-face notes, physician orders, discharge summaries, history and physicals, and other supporting records.
Communicate with patients, families, physicians, referral sources, and internal staff regarding referral status and coordination.
Enter referral and patient information accurately into the electronic medical record system.
Assist with coordinating start-of-care scheduling and communicating updates to clinical staff.
Track pending referrals and follow up timely on missing information or documentation.
Answer incoming phone calls and direct calls to the appropriate department or staff member.
Assist with general office tasks such as scanning, faxing, filing, copying, uploading documents, and maintaining organized records.
Support administrative staff with data entry, reports, employee paperwork, patient files, and other office needs.
Assist with maintaining communication between office staff, field clinicians, patients, and referral sources.
Help ensure office workflow is organized and that pending items are followed up on timely.
Maintain patient confidentiality and follow HIPAA requirements at all times.
Perform other administrative duties as assigned by management.

Qualifications

Previous experience in home health, hospice, medical office, healthcare administration, intake, scheduling, or administrative support preferred.
Nursing license is not required.
Strong communication and customer service skills.
Ability to multitask, stay organized, and prioritize work in a fast-paced office environment.
Strong attention to detail and follow-through.
Comfortable using computers, email, fax systems, electronic medical records, and basic office software.
Must be professional, dependable, and able to work well with both clinical and administrative staff.
Ability to maintain confidentiality and handle sensitive patient information appropriately.

Preferred Experience

Home health or hospice office experience.
Intake, referral coordination, scheduling, or medical office experience.
Experience communicating with hospitals, physician offices, insurance companies, and patients.
Familiarity with home health documentation requirements is a plus.
Bilingual English/Spanish is a plus, but not required.

Schedule

Full-time position
Monday through Friday
Occasional flexibility may be needed based on office needs.
