Job Overview

Rockwood Retirement Communities is seeking a dedicated and experienced Social Services Director to oversee the South Hill Campus, leading our behavioral health and social service programs. This position works with considerable autonomy and is responsible for providing professional services to individual residents, families, care team, other departments, and the community. The Social Services Director leads the delivery of social services and psychosocial support for residents across all levels of care within our Life Plan Community. This role assesses resident needs, develops individualized care plans, coordinates community resources, supports residents and families through transitions, and collaborates with interdisciplinary teams to promote quality of life, resident rights, and regulatory compliance. The director also ensures compliance with state and federal regulations, and fosters a compassionate, resident-centered environment.

Pay Range: $40-$43.50 per hour DOE

Shift type: Full-time (Flexible hours).

Responsibilities

Providing mental health support to residents and/or team members with issues such as stress management, family problems, grief, relationship issues, self-esteem issues, and anger management
Ensuring that the organization’s programs are aligned with community needs and priorities
Coordinating services with other community agencies and organizations that serve the same population
Maintain work operations by following policies and procedures
Maintain confidentiality of all resident information
Adhere to all professional standards, policy and procedures, federal state and local requirements
Work with all supervisors and staff and maintains a good working relationship with all staff
Uphold the RRC Values in all interactions
Complete admissions, quarterly, annual, and significant change psychosocial assessments.
Develop, implement, review, and update individualized resident care plans in collaboration with the interdisciplinary team.
Assess residents' cognitive, emotional, behavioral, psychosocial, and decision-making status, including identifying concerns related to depression, anxiety, suicide risk, abuse, neglect, self-neglect, exploitation, and changes in mental status.
Provide crisis intervention, emotional support, mental health support, and advocacy to residents and families throughout about changes in health status, transitions of care, hospice services, and end-of-life planning.
Coordinate and facilitate family meetings, care conferences, and interdisciplinary meetings to support resident-centered care planning.
Collaborate with physicians, nurse practitioners, behavioral health providers, psychologists, therapists, pharmacies, hospice agencies, home health agencies, and community resources to coordinate resident care.
Monitor residents with behavioral health conditions, dementia-related behaviors, and other psychosocial concerns by developing behavioral interventions, documenting behavioral trends, coordinating psychiatric and neurological consultations, and supporting non-pharmacological approaches whenever appropriate.
Monitor psychotropic medication use, participate in psychotropic medication review meetings, advocate for appropriate diagnoses and gradual dose reductions when clinically indicated, and communicate recommendations with providers.
Prepare behavioral summaries, psychosocial documentation, progress notes, assessments, and regulatory documentation in accordance with state and federal requirements.
Investigate resident concerns, complaints, and grievances while promoting resident rights, dignity, autonomy, and person-centered care.
Participate in quality assurance, quality improvement, emergency preparedness planning, survey readiness, and regulatory compliance activities, including development of tracking systems and performance improvement initiatives.
Develop, implement, and revise departmental policies, procedures, and documentation processes to improve quality of care and regulatory compliance.
Provide education, coaching, orientation, and ongoing support to staff regarding dementia care, resident rights, communication techniques, professional boundaries, behavioral interventions, and psychosocial best practices.
Serve as a resource to leadership and the interdisciplinary team by assisting with complex ethical situations, behavioral concerns, discharge planning, capacity concerns, and resident advocacy.
Maintain tracking systems for high-risk residents, behavioral health needs, emergency preparedness, care conferences, and other quality improvement initiatives.
Perform additional clinical, administrative, leadership, and regulatory responsibilities necessary to support resident care and organizational operations.
Level 1 Caregiving for Mental Health and Dementia Instructor
Perform all other duties as assigned

Job Qualifications

Bachelor's degree (B.A.) required, MSW preferred
Must have 2 years’ experience in either hospital skilled nursing facility or other related areas
Social Services: 1 year (preferred)
Long Term Care: 2 years (preferred)
Detail oriented with the ability to multi-task
Exceptional time management and organizational skills
Ability to read, write and comprehend the English language
