Full Job Description

Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting Previously Homeless individuals providing guidance, resources, and advocacy. This position requires a strong understanding of workforce development, behavioral health, and social services to effectively assist clients in navigating their challenges and achieving their goals.

Duties

Conduct comprehensive assessments to identify client needs and develop tailored service plans.
Provide crisis intervention and management to clients facing immediate challenges.
Facilitate access to supportive housing, healthcare services (including Medicare), recovery, treatment, employment, and leisure.
Offer mentoring and supervision to clients, empowering them through skill-building activities.
Collaborate with community organizations and service providers to enhance client support networks.
Maintain accurate documentation of client interactions, progress, and outcomes in compliance with organizational policies.
Monitor client progress and adjust service plans to ensure effective support.
Advocate for clients' rights and needs within various systems, including social work frameworks.

Experience

Proven experience in case management or related fields, such as social work or behavioral health, is preferred.
Familiarity with workforce development programs and crisis intervention techniques is highly desirable.
Experience working with diverse populations, including children and families, is a plus.
Strong interpersonal skills with the ability to build rapport with clients from various backgrounds.
Excellent organizational skills and attention to detail for managing multiple cases simultaneously.
Ability to work collaboratively within a team environment while also demonstrating independence in managing caseloads.
