Coordinated Entry Assessment Specialist

Salary Range: $44,000 - $46,000

REPORTS TO: Supervisor of Direct Support

About Us: The Long Island Coalition for the Homeless (LICH) is dedicated to addressing homelessness on Long Island by collaborating with non-profits, government agencies, and community partners. As a leading advocate and service provider, we work to support individuals and families experiencing homelessness through a range of programs and initiatives. Our efforts include direct assistance, outreach, resource distribution, and emergency support. Program Summary – Coordinated Entry (CE): The Coordinated Entry (CE) program supports individuals and families experiencing or at risk of homelessness on Long Island through housing assessments, resource coordination, diversion support, and referrals to shelter and supportive services. The program works collaboratively with community partners, outreach teams, and housing providers to ensure a coordinated, person-centered, and trauma-informed response to homelessness while helping households access stable housing and essential resources as quickly as possible.

Position Summary: The Coordinated Entry Assessment Specialist is responsible for conducting assessments for individuals and families experiencing homelessness on Long Island. The role will require a combination of office and fieldwork and will be expected to be highly mobile. This role involves working closely with clients experiencing homelessness to conduct housing needs assessments in places such as shelters, unsheltered, in the community or over the phone to determine their needs and connect them with appropriate services and resources through the Coordinated Entry System. This role will also monitor and respond to helpline calls within business hours, providing immediate support, information, and referrals to individuals experiencing or at imminent risk of homelessness.

Key Responsibilities:

Conduct comprehensive assessments with targeted individuals and families experiencing homelessness in shelter, on the street or places not meant for human habitation using established localized assessment tool and protocols.
Aid with short term housing problem solving and resource connections (diversion) for people that are experiencing homelessness, including developing individualized service plans based on client needs, strengths, and preferences related to housing.
During business hours, answer and respond to helpline calls within 48 hours of message being left from individuals experiencing or at risk of homelessness.
Provide information and referrals to callers and households being assessed, ensuring they are connected to the most appropriate services.
Uphold and ensure privacy and confidentiality for all clients and persons experiencing or at risk of homelessness
Track call volume and trends, prepare and submit timely reports on assessment outcomes, helpline activity, and client progress.
Triage calls and referrals as appropriate to crisis, health, outreach, and case management supports.
Participate in CE Team meetings, All Agency Meetings, and other meetings relevant to job scope as approved by supervisor and work collaboratively with a multidisciplinary team, including outreach workers, case managers, and housing providers, to ensure a coordinated response to homelessness.
Input and maintain accurate and up-to-date client information in the Homeless Management Information System (HMIS).
Provide opportunities for new staff and interns to observe day-to-day tasks to help them learn about the role.
Collaborate with other service providers and agencies to facilitate access to shelter, housing, and other supportive services.
Assist in the development and implementation of policies and procedures related to coordinated entry and helpline operations.

Qualifications & Experience: Must have a commitment to organization’s mission and goals. Must embrace a person centered and trauma-informed approaches. Have strong knowledge of community resources, connections, housing options and navigating the homeless system. Must be able to work effectively with diverse people and personalities and promote inclusive environments/spaces; Must be able to work as a part of a team; should possess strong organizational skills and time management skills; possesses effective verbal and written communication skills; possess ability to remain calm and focused under pressure; have ability to use or learn to use computer database applications. Must have a clean driving record to operate agency vehicles.

Work Location & Travel: This position is field-based and requires travel throughout Nassau and Suffolk Counties. Staff are expected to be mobile and may spend the majority of the workday in the community conducting outreach, transporting participants, or attending appointments.Agency vehicles are available for program use. A valid driver’s license and ability to drive are required.

Work Location & Schedule: Work Location: This role is primarily on-site and requires regular attendance at our office. Limited flexibility for a hybrid work arrangement may be considered after successful completion of the 90-day probationary period. Hybrid or remote work is not guaranteed and must be approved in advance by your supervisor/manager based on program needs, job responsibilities, and individual performance. All remote work is subject to ongoing review, and any concerns related to responsiveness, availability, productivity, or misuse of remote flexibility may result in the adjustment or revocation of remote privileges at any time. Employees working remotely are expected to:

▪ Be available and responsive during their scheduled work hours

▪ Communicate proactively with supervisors and teammates in a timely manner

▪ Complete work assignments in a timely and satisfactory manner
