Full-Time

$45.00 – $50.00 per hour (DOE)

As a Hospice Social Worker, you help patients and families navigate the emotional and practical realities of end-of-life care.

Provide psychosocial assessments and ongoing support
Help patients and families navigate difficult decisions

What You’ll Be Doing

Conducting psychosocial assessments in patients’ homes
Developing and implementing care plans
Supporting patients and families with emotional and practical needs
Educating on advance directives and care planning
Coordinating with community resources and services
Participating in interdisciplinary team meetings
Providing support across hospice (with occasional home health collaboration)

What Helps You Succeed

Master’s degree in Social Work (MSW)
Licensed Clinical Social Worker (LCSW) in Oregon
2+ years healthcare experience preferred (hospice or home health a plus)
Current BLS certification
Strong communication and organizational skills
Valid driver’s license and reliable transportation

This role includes travel throughout Jackson County and surrounding areas, providing care in homes and facilities.
