Licensed Social Worker (LSW) | Mission Valley Nursing and Transitional Care | Mission, TX | Skilled Nursing/Long-Term Care | Full-Time

Are you a Licensed Social Worker with a passion for supporting residents, families, and interdisciplinary care teams in a skilled nursing setting? If so, we would love to speak with you.

Wellsential Health is currently seeking a Licensed Social Worker (LSW) to support Mission Valley Nursing and Transitional Care. This role is an important part of the facility team and helps support admissions, resident advocacy, psychosocial needs, care planning, family communication, and resident quality of life.

What You'll Do

As the Licensed Social Worker, you will work closely with residents, families, responsible parties, facility leadership, and the interdisciplinary team to identify resident needs, provide support, connect residents with resources, and help ensure the mental, emotional, and social well-being of each resident.

Through communication, advocacy, and follow-through, the Licensed Social Worker helps residents feel heard, supported, and respected while assisting with admissions, care planning, concerns, psychosocial needs, and transitions in care.

Key Responsibilities

Develop relationships with residents and responsible parties to support admissions, counseling, and identification of resident needs.
Support residents and responsible parties with social interaction, reality orientation, intellectual stimulation, emotional support, and adjustment to the facility environment.
Assist the Business Office Manager with resident financial concerns as needed while helping promote emotional security for residents.
Complete acute care plans as needed and attend care plan meetings to support resident-centered planning and communication.
Assist the Administrator with resident, family, and responsible party concerns, including follow-up and concern management.
Complete initial, routine, and as-needed psychosocial and socialization assessments, and refer residents for additional professional psychosocial services when appropriate.
Discuss advance care planning with the responsible party upon admission and as required by state regulations, and complete necessary documentation.
Refer residents for vision, dental, hearing, and other community or support services as needed.
Assist with room changes by communicating with the resident and responsible party and completing required forms.
Make morning rounds prior to the morning meeting and provide department updates during daily department head meetings as needed.
Attend committee meetings as required and complete monthly QA reports.
Perform suicidal and homicidal assessments and complete required follow-up in accordance with facility policy and applicable requirements.
Assist in investigations as requested by the Administrator.
Assist with completion of the MDS as needed.
Work rotating weekend Manager on Duty schedule as assigned.
Complete other duties as assigned or needed.

What We're Looking For

Bachelor's or Master's degree in Social Work required.
Must be a Licensed Social Worker in the state of Texas in good standing.
Previous experience working in a skilled nursing facility, long-term care, healthcare, or senior living setting preferred.
Knowledge of resident rights, psychosocial assessments, care planning, advance care planning, and discharge or community resource coordination preferred.
Ability to communicate professionally with residents, families, responsible parties, facility leadership, nursing, business office, and interdisciplinary team members.
Strong organization, documentation, follow-up, problem-solving, and concern-management skills.
Ability to support residents and families with compassion, professionalism, confidentiality, and respect.
Must be eligible to work in the United States.
