Social Worker / Case Manager – Hybrid (Community Transition Services, 9/80 Schedule)

Location: Hybrid / Remote – Mid-Michigan
Schedule: Full-time | 9/80 schedule (one day off every other week, based on satisfactory performance)
Salary: $50,000–$60,000 depending on license and experience

Position Summary

The Social Worker /Transition Navigator provides transition navigation services, including assessments, service linkage and coordination, discharge planning, monitoring, and documentation. This role requires independent time management, professional judgment, and consistent performance to maintain hybrid work flexibility.

Basic Job Duties

Conduct psychosocial assessments, reassessments, and provide Community Transition Services to integrate individuals back into the community
Provide and coordinate case management services in accordance with the individual person-centered transition plan
Complete accurate and timely documentation in compliance with State, program, and regulatory requirements
Communicate effectively with office staff, participants, nursing home staff, and community agencies
Conduct required in person visits within the assigned service area

Work Environment

Hybrid role: work from home with limited in-office presence (generally one day per week)
Field-based client visits are required; mileage reimbursement provided for assessment-related travel
Flexible scheduling supported through a 9/80 work schedule contingent upon satisfactory job performance
Ongoing performance expectations related to productivity, documentation, and quality outcomes

Requirements

· Social Worker licensed in the State of Michigan (LLBSW, LBSW, LLMSW, or LMSW) or

· Registered Nurse licensed in the State of Michigan, or

· Non-licensed or other licensed health care professionals with the following qualifications:

a) A bachelor’s degree in a health or human services field or Community Health Worker certification, and

b) At least three years of experience in the provision of health or social services.

Understanding of confidentiality and HIPAA regulations
Strong interpersonal, organizational, and communication skills
Ability to work independently while meeting productivity and quality standards

Preferred Qualifications

One year or more of social work or case management experience
Experience in home health care, health and human services, or community-based programs
Knowledge of healthcare regulations and quality management principles
