Job Overview

We are seeking a dedicated and knowledgeable Community Pharmacist to join our healthcare team. In this vital role, you will serve as a key healthcare provider within the community, ensuring safe and effective medication management, patient education, and compliance with all regulatory standards. Your expertise will directly impact patient outcomes by delivering personalized care, managing complex medication therapies, and fostering trust within the community. This position offers an exciting opportunity to utilize your clinical skills in a dynamic environment committed to exceptional patient service and healthcare excellence.

Duties

Dispense prescriptions accurately while ensuring compliance with all federal and state pharmacy laws, including HIPAA and pharmaceutical regulatory standards such as USP 797 for sterile compounding.
Provide comprehensive patient assessments to determine appropriate medication therapy, including evaluating medical histories, physiology, anatomy, and current health conditions.
Counsel patients on medication usage, potential side effects, interactions, and proper storage to promote optimal health outcomes.
Manage inventory effectively by monitoring stock levels, ordering medications, and ensuring proper storage conditions in accordance with pharmacy regulations.
Prepare compounded medications using aseptic techniques when necessary for specialized treatments such as IV infusions or pediatric care.
Administer immunizations and participate in vaccination programs following clinical guidelines and safety protocols.
Utilize pharmacy software systems (PIS) and electronic health records (EHR) to document patient interactions, medication histories, and clinical notes accurately.
Collaborate with healthcare providers on medical management plans, pain management strategies, and acute care cases to ensure seamless patient care delivery.
Maintain strict adherence to pharmacy information systems (PIS), drug regulation policies, and pharmacy confidentiality policies to uphold legal and ethical standards.
Support pharmacy technicians in daily operations while supervising their work to ensure accuracy and compliance.

Qualifications

Valid pharmacist license with active registration in the relevant jurisdiction.
Proven experience in community pharmacy settings with knowledge of LTC (Long-Term Care) pharmacy practices and hospital or acute care environments preferred.
Strong clinical pharmacy experience with familiarity in critical care, pediatrics, immunizations, pain management, and medical terminology.
Experience with pharmacy residency programs or advanced clinical training is highly desirable.
Proficiency in pharmacy software systems (PIS), EMR (Electronic Medical Records), EHR systems, and inventory management tools.
Knowledge of basic life support (BLS), aseptic techniques such as USP 797 standards, IV insertion, infusion procedures, and drug regulation policies.
Excellent patient service skills with the ability to conduct thorough patient assessments and medication administration counseling.
Familiarity with healthcare compliance standards including HIPAA regulations and clinical confidentiality policies.
Ability to work effectively within a multidisciplinary team in fast-paced environments such as hospital or acute care settings.
