Summary: This (FLSA Non-exempt) position has the responsibility to coordinate all Discharge Services for mental health, substance abuse, and developmental disabilities individuals incarcerated locally. Employee is also responsible to include assistance with Re-Investment clients, performing Restoration in and out of our local jails, be a liaison with ESCSB's NGRI Program, along with providing training programs CPR & TOVA.
General areas of responsibilities include:
- Provide discharge planning for consumers by linking individual service needs with appro-priate resources within the agency, other human service agencies in the community, public and private hospitals, and for project funded or state hospital admitted consumers;
- Develop housing resources for the seriously mentally ill population of the Eastern Shore;
- As assigned, planning for and providing Forensic based-case management services for per-sons experiencing mental health, developmental disabilities, and/or substance abuse popula-tion;
- As assigned oversee and provide being a liaison between ESCSB and Region V Reinvest-ment Project facilities for persons sent to those facilities as an ESCSB Crisis Intervention, per the Region V Reinvestment Project Policies and Guidelines; and
- Providing training programs to include CIT, CPR, & TOVA instruction.
In carrying out the position responsibilities, the incumbent reports to and receives supervision from the Supervisor of Transitional/Forensic Programs and Training. The incumbent is required to exer-cise sound judgment, demonstrate initiative, maintain confidentiality, sustain effective intra- and inter-organizational working relationships, and develop and maintain sound, qualitative, well-organized services for the specified populations. The incumbent performs in accordance with es-tablished ESCSB policies and procedures, demonstrates initiative, exercises sound judgment, main-tains confidentiality of information, and sustains effective working relationships.
Essential Functions:
- Provide linkage and coordination between hospitals, jail and community resources in order to accomplish stable community placement for consumers discharged to the Eastern Shore catchment area from project funded or state hospital admissions;
- Provide outreach services to consumers for the purpose of locating, developing or obtaining needed resources for community placements;
- Complete and maintain proper case documentation in accordance with all licensure, Medi-caid, and third party insurances;
- Attend meetings and in-service training activities (inter- and intra agency staffing, confer-ences, seminars, etc.) related to position responsibilities;
- Perform other job-related duties as assigned by the Supervisor of Transitional/Forensic Pro-grams and Training and/or the Director of Clinical Services or Executive Director;
- Interview individuals with SMI and convey their voices to the jail staff orally and/or in writ-ten reports;
- Perform brief jail mental health screens;
- Conduct comprehensive assessments to determine an individual's emergency needs, func-tioning level, and the level of services needed to provide the appropriate degree of intensity in the least restrictive environment;
- Interview collateral contacts and gather and review relevant criminal, academic, medical, mental health, police, corrections and other documentation; prepare and write concise, rele-vant reports as required;
- Develop and oversee person-centered treatment plans and communicate with the E.S. Fo-rensic Discharge Planning Unit team members;
- Serve as the liaison between incarcerated individuals;
- Coordinate services and treatment planning with other treatment providers working with the individual;
- Reassign a caseload from the local jails, for which they will perform community-based wellness checks and
- Administer the Daily Living 20 (DLA20) outcome measurement tool designed to measure the daily living areas impacted by serious mental illness.
Region V Reinvestment Project Liaison
- Work with the emergency services clinicians while under the supervision of the Supervisor of Transitional/Forensic Programs and Training who facilitate an individual’s admission to a psychiatric hospital or an in-patient crisis stabilization facility under Region V Reinvestment funding. This includes receiving referrals, reviewing assessments and on-going treatment, and gathering background information relevant to the individual's treatment needs;
- Provide the direct liaison between the ESCSB and the treating facility which includes mak-ing on-site visits (initial visit must be within 24 hours of admission) to the facility to con-duct on-going assessments of treatments needs, discharge planning, setting up the appropri-ate level of services in the community when individual is discharged, and monitoring these follow-up services, at least through the first appointments;
- Link (refer) the individual to other services that support the goals and objectives of the ISP;
- Make collateral contacts on behalf of or with an individual;
- Provide pre-discharge planning and follow-up services for individuals who have recently been discharged from public and private hospitals in order to provide a smooth transition from hospital to community;
- Enhance the individual's integration into the community by helping the individual learn about and use services that increase his/her independence, and support his/her positive and effective involvement in the community;
- Monitor the provision of all identified services to assure that they are provided as planned, that the individual is able to use the services, and to make sure provision of services is con-sistent with the ISP; and
- Educate and counsel in the context of helping an individual use community resources and increasing his/her problem-solving abilities relative to accessing and using needed services.
NGRI State Hospital Liaison
- Coordinate with SW/DBDHS/ESCSB Clinical Director and Executive Director on NGRI cli-ents in state hospitals while under the supervision of Supervisor of Transitional/Forensic Programs and Training;
- Develop DC Plans for NGRI Clients; and
- Attend DC Manager and Liaison DC meetings.
NGRI/Restoration Worker
- Coordinate with local jails, courts, ESCSB Supervisor of Transitional/Forensic Programs and Training, Clinical Director/Executive Director, lawyers, independent evaluators and DBDHS cases;
- Restore possible cases in the local jails; and
- Complete all paperwork involved with cases and disseminate to all involved entities.
General Duties
- Attend meetings and in-service training activities (inter- and intra agency staff meetings, conferences, seminars, etc.) related to position responsibilities;
- Provides CPR training and TOVA instruction;
- Provide documentation of services provided per Medicaid, licensure, other 3rd party pay-ers, and ESCSB policies and procedures and;
- Perform other job-related duties as directed by the Supervisor of Transitional/Forensic Pro-grams and Training, Clinical Director, and/or Executive Director.
Related Duties and Responsibilities

Education and Experience:
- Bachelor’s degree in psychology, social work or related field required, minimum six (6) months position related experience required; Virginia licensure as a Qualified Mental Health Provider required; or any equivalent combination of education, training, and experience which provides the required knowledge and abilities.
Required Knowledge, Skills and Abilities:
The incumbent should possess the following knowledge, skills, and abilities:
- The nature of serious mental illness in adults, treatment modalities and intervention tech-niques, such as behavior management, independent living skills training, supportive coun-seling, family education, crisis intervention, discharge planning and service coordination; housing regulations and resources; budget development and review; consumers' rights; local community resources and service delivery systems; including support services (e.g. housing, financial, social welfare, dental, educational, vocational, legal/advocacy), eligibility criteria and intake processes, termination criteria and procedures, and generic community resources (e.g. churches, clubs, self-help groups); types of mental health programs and services; ef-fective oral, written and interpersonal communication principles and techniques; general principles of record documentation, the service planning process and major components of a service plan.
- Identifying and documenting a consumer's need for resources, services and other supports; using information from assessments, evaluations, observation and interviews to develop service plans; identifying services within the community and established service system to meet the Individual's housing needs; identifying grant opportunities in keeping with ESCSB priorities; writing grants that meet ESCSB prioritized needs.
- Promote ESCSB Forensic-based housing on local and regional level; educate community on ESCSB housing resources; demonstrate a positive regard for consumers and their families (e.g. treating consumers as individuals,); be persistent and remain objective; work as a team member, maintaining effective inter- and intra- agency working relationships; communicate effectively; verbally and in writing; establish and maintain ongoing supportive relationships; research grant opportunities; write grants.
- The incumbent must have a valid driver's license and satisfactory driving record.
