Case Manager

Reports To: MI Community Director

Position Summary

Are you passionate about helping individuals achieve recovery and improve their quality of life? We are seeking a compassionate, organized, and motivated Case Manager to join our team. In this role, you will work closely with individuals experiencing mental illness or co-occurring mental illness and substance use disorders, connecting them with resources, coordinating care, and advocating for their success. Minimum Qualifications

Bachelor's degree, preferably in a Social Science field
Valid driver's license with a good driving record
Completion of Alabama Department of Mental Health (ADMH) approved Case Management Training

Essential ResponsibilitiesProvide Case Management Services

Collaborate with community partners, including hospitals, DHR, and other agencies, to identify and coordinate services.
Contact individuals referred to the program within 72 hours of referral.
Complete eligibility screenings and admissions or refer individuals to appropriate services based on assessed needs.
Provide comprehensive case management services, including:
Care coordination and service linkage
Crisis intervention
Advocacy
Needs assessment
Transportation or transportation coordination
Coordinate smooth transitions between levels of care based on ongoing assessments.
Ensure individuals have access to structured, person-centered case management and recovery support services.
Monitor implementation of each person's plan of care by:
Evaluating progress toward goals
Determining whether services remain appropriate
Identifying needed modifications to the treatment plan
Monitor risk factors and address special needs as they arise.
Encourage active participation in continuing care and recovery activities.
Complete ongoing assessments, service evaluations, and care planning in accordance with ADMH and Medicaid requirements.
Complete all clinical and administrative documentation within one working day of service delivery.
Maintain accurate, concise, and timely documentation that meets Center and ADMH standards.
Exercise sound professional judgment when making referrals.
Provide consultation and support to community agencies as appropriate.

Community Outreach & Education

Represent the Center professionally through community presentations and outreach activities.
Tailor educational presentations to meet the needs of diverse audiences.
Serve as a positive advocate for mental health services within the community.

Professional Development

Participate in workshops, in-service training, and Quality Assurance activities.
Stay current with professional literature and best practices.
Seek consultation and collaborate effectively with colleagues.

Team Collaboration

Maintain positive and productive working relationships with support staff.
Communicate accurate and timely information to team members to ensure quality service delivery.

Additional Responsibilities

Perform other duties as assigned by the immediate supervisor.
Complete all assigned responsibilities accurately, professionally, and within established timeframes.
Maintain confidentiality and treat all assigned duties with the same level of priority and professionalism as core job responsibilities.
