Full Job Description

Overview
We are seeking a highly motivated clinician to join our team to care for patients in our endocrinology clinic with a focus on diabetes education. As part of our ADA Recognized Diabetes Education Program, you will be working primarily with our endocrinology team (physician, NP, PA and other staff) taking care of adults with Type I & Type II diabetes. This position will be focused on the education and management of patients to improve clinical outcomes and quality of life.

Key Responsibilities

Patient Education: Teach individuals or groups how to use blood sugar monitors, continuous glucose monitors (CGMs), and insulin pumps.
Care Planning: Build custom meal plans, adjust activity routines, and create plans for handling high or low blood sugar.
Medication Training: Instruct patients on how to safely take oral medications or self-administer insulin and non-insulin injections.
Behavioral Support: Help patients overcome the emotional and lifestyle challenges of living with a chronic illness.

· Data Tracking: Download and analyze patient data (blood glucose logs, pump and sensor downloads) to track progress and discuss changes with medical providers.

Requirements

Bachelor’s degree from an accredited institution.
Registered Dietitian Nutritionist (RDN) or Registered Nurse (RN) with Tennessee state licensure.
Strong skills in patient assessment and the ability to develop effective diabetes specific care plans.
Must be comfortable utilizing advanced technology (Electronic Medical Records, Microsoft Office, device data/downloads)
Familiarity with blood glucose management, insulin provision, and adaptable education techniques.
Excellent communication skills, both verbal and written, with the ability to educate diverse patient populations.
Ability to work collaboratively within a multidisciplinary team environment.
Passion for promoting health and quality of life in people with diabetes.

Preferred Qualifications

3+ years of experience working with diabetes self-management
Certified Diabetes Care and Education Specialist (CDCES) or willing to obtain when eligible
