Full Job Description

Summary

The Care Manager, Retention is responsible for developing and maintaining relationships with clients and caregivers to ensure retention, compliance, and quality-of-care. This role will manage long-term client relationships and satisfaction, which includes conducting home visits, monitoring EVV compliance and managing EVV barriers, Plan of Care annual updates, Caregiver evaluations (90 day and annual), incident reporting, post-hospitalization follow-up/Care Gaps, and other duties as assigned. Strong communication, problem-solving skills and initiative to be present in the community are necessary for this role to be successful. The Care Manager, Retention reports to the Associate Director of their Office.

Responsibilities

Supervise a team of caregiver employees on a daily basis, which includes pre-billing responsibilities and timesheet collection, conducting 90-day and annual performance reviews, coaching and disciplinary action, and ECNF completion.
Monitor client eligibility, authorizations, and hours utilization to ensure maximum service delivery. Address all authorization/eligibility issues same day as notified.
Reconcile and complete dashboard and pre-billing duties daily and timely for weekly payroll processing, as needed, per Office structure and needs.
Care coordination - schedule, fill open shifts and ensure proper staffing of cases.
Fill open shifts to ensure no disruption in client services.
Ensure EVV and timesheet compliance.
Monitor Plan of Care (POC) compliance.
Complete home visits for all clients on caseload.
Recruit new clients as needed to meet Budgeted goals.
Assist with marketing efforts, as assigned, and perform outreach functions including attending health fairs, community events, and other activities, as required.
Provide on-call coverage, as scheduled.

Required Education and Experience

Bachelor’s Degree in Human Services, Business Administration, or related field preferred or minimum 2 years’ experience in Home Care, Social Work, and/or work with The Office of Long-Term Living.
Proficient in Microsoft Office: Word, Excel, Power Point, Outlook.
Knowledge of HHAeXchange, SAMS, and/or HCSIS desirable.
Experience with case management/coordination, records/utilization management, preparing reports, or other documentation desirable.
Spanish Speaking preferred
Must have own car, valid driver’s license, and insurance.

Physical Requirements

May require sitting, standing, or walking on a level surface for periods throughout the day. May require lifting up to 20 pounds. Regional travel up to 50% required to other JEVS locations, home visits and other related functions.

The base pay for this position ranges from $45,000 to $52,000 and will vary based on factors such as skills and experience. Base pay is one part of the Total Compensation that JEVS Care at Home provides to recognize employees for their work.
