POSITION TYPE: FULL-TIME Hourly Position- This is a Work from Home Position that requires 2- 3 days in the field completing assessments in our participant’s homes
POSITION TITLE: SERVICE COORDINATOR

JOB SUMMARY: Coordinates, develops and monitors the assessment, care planning process of all aspects of the consumer needs. Serves as an advocate and liaison on behalf of consumers.

DUTIES TO INCLUDE BUT NOT LIMITED TO:

Complete intake enrollment/assessments, reassessments, and monitoring
Monitor participant's health, safety and welfare
Provider members with all information related to receiving waiver services, including list of providers (for services), freedom of choice, rights, responsibilities and liabilities
Collaborate with the consumers to meet their needs & goals
Develop an Individual Service Plan (ISP) using Person Centered Planning strategies
Maintain confidentiality of consumers information.
Provide service in accordance with existing policies, procedures and regulations from the company.

EDUCATIONAL REQUIREMENTS:Service Coordinators must, have a Bachelor’s degree in social work, psychology, or other related fields or 3 years of Case Management or other Human Services work.
