NexStep Health is a growing healthcare company focused on helping patients recover, manage chronic conditions, and live safely at home. We provide personalized, hands-on support to help patients and families navigate an often complex healthcare system.

NexStep Health provides Medicare Chronic Care Management (CCM), private-duty nursing, Certified Home Health Aide (CHHA) services, and home-based clinical support.

Job Summary

NexStep Health is seeking an experienced and compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team as a Nurse Care Manager.

The Nurse Care Manager will support patients enrolled in NexStep’s Medicare Chronic Care Management program. The role includes patient and caregiver engagement, care planning, health education, symptom and medication monitoring, coordination with healthcare providers, and helping patients follow through with recommended care.

The Nurse Care Manager may also support patients during transitions of care and through NexStep’s home-based care programs. This position includes a combination of office-based work, telephone outreach, and field-based visits in patients’ homes and other community settings.

This position is ideal for a nurse who enjoys building ongoing relationships with patients and families and wants to provide meaningful care outside of a traditional bedside setting.

Key Responsibilities

Patient Engagement and CCM Enrollment

Educate eligible patients and caregivers about Medicare Chronic Care Management and NexStep’s services
Enroll patients referred by partner practices, healthcare providers, and other referral sources, with a target of approximately 10–15 new enrollments per week
Develop trusting, long-term relationships with patients and caregivers
Build and manage an assigned panel of CCM patients
Complete regular patient outreach by telephone, video, and occasional in-person visits

Chronic Care Management and Care Coordination

Complete initial assessments and develop individualized, patient-centered CCM care plans within the nurse’s scope of practice
Engage enrolled patients regularly and provide approximately 40–60 minutes of care management support per month, based on patient needs
Review and update care plans as patients’ conditions, goals, and care needs change
Monitor patients’ health status, symptoms, medications, care needs, and progress toward established goals
Provide education regarding chronic conditions, medications, lifestyle recommendations, preventive care, and self-management strategies
Identify changes in condition and communicate concerns to the appropriate provider or supervising clinician
Coordinate care among primary care providers, specialists, hospitals, home health providers, pharmacies, and other members of the patient’s care team
Assist patients with scheduling follow-up appointments, obtaining records, and communicating with healthcare providers
Support patients following hospitalizations, emergency department visits, procedures, rehabilitation stays, and other transitions of care
Identify barriers that may prevent patients from following through with recommended care
Connect patients and families with appropriate healthcare, social, and community resources
Encourage patients to complete recommended appointments, testing, treatments, screenings, and preventive care
Document all CCM activities and track time in accordance with Medicare and program requirements

Home and Community-Based Support

Conduct scheduled visits in patients’ homes and other community settings, as needed
Identify patient and caregiver needs, care gaps, and potential home safety concerns
Support patients and families in arranging appropriate home-based services
Collaborate with home health aides, nurses, providers, and family caregivers to promote safe and coordinated care at home
Assist with other NexStep clinical and care coordination programs as needed

Documentation and Program Responsibilities

Maintain timely, accurate, and complete documentation in NexStep’s care management and electronic health record systems
Follow established clinical, privacy, documentation, and escalation protocols
Participate in team meetings, patient case reviews, workflow improvement, and program development
Assist with the continued development of NexStep’s CCM and home-based care programs

Qualifications

Active New Jersey Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in good standing
Three or more years of direct patient care or clinical experience preferred
Experience in Chronic Care Management, case management, care coordination, population health, home health, or chronic disease management is a plus
Experience working with older adults, medically complex patients, and family caregivers strongly preferred
Experience in home health, ambulatory care, primary care, rehabilitation, skilled nursing, or community-based healthcare is helpful
Comfortable working independently in both office and field-based settings
Experience using electronic health record or care management systems
Excellent communication and interpersonal skills, with the ability to engage patients and caregivers effectively
Strong organizational, critical-thinking, and problem-solving skills
Ability to manage multiple priorities in a fast-paced, service-oriented environment
High level of professionalism and discretion when handling confidential information
Reliable transportation and the ability to travel to patient homes and community locations within the assigned service area
