Full Job Description

Position Summary

12 and Beyond Missouri is seeking a compassionate, dependable, and experienced Case Manager to join our drug and alcohol treatment team. The Case Manager will work directly with individuals receiving treatment for substance use disorders (SUD) and co-occurring mental health conditions, helping clients identify their needs, access appropriate resources, overcome barriers, and successfully progress through treatment and recovery.

The ideal candidate will have experience in behavioral health, addiction treatment, mental health, social services, or a related setting and be comfortable working collaboratively as part of a multidisciplinary treatment team.

Essential Duties and Responsibilities

Provide case management services to clients receiving substance use disorder treatment.
Complete client needs assessments and assist in developing individualized case management plans.
Identify barriers that may interfere with treatment participation, recovery, stability, or successful discharge and work with clients to develop solutions.
Connect clients with appropriate community resources and support services.
Assist clients with accessing housing, transportation, employment, education, medical care, mental health services, legal resources, benefits, and other community supports.
Work effectively with individuals experiencing substance use disorders and co-occurring mental health concerns.
Maintain regular client contact and complete accurate and timely documentation of case management services.
Collaborate with counselors, therapists, medical providers, peer support specialists, and other members of the multidisciplinary treatment team.
Participate in treatment team meetings and communicate relevant information regarding client needs, barriers, progress, and discharge planning.
Assist with discharge and aftercare planning to support successful transitions to the next level of care or independent/community living.
Coordinate services with outside agencies, probation/parole officers, courts, healthcare providers, treatment programs, and community organizations when appropriate and properly authorized.
Advocate for clients while promoting independence, accountability, personal responsibility, and healthy recovery skills.
Maintain appropriate professional boundaries and protect client confidentiality.
Follow organizational policies and all applicable state and federal confidentiality requirements.
Perform additional case management duties as assigned.

Qualifications

Previous experience in substance use disorder treatment, behavioral health, mental health, case management, social services, or a related field preferred.
Experience working with individuals with substance use and co-occurring mental health disorders strongly preferred.
Knowledge of addiction, recovery, relapse prevention, and community resources.
Ability to work effectively with individuals from diverse backgrounds and with complex needs.
Strong communication, organization, documentation, problem-solving, and time-management skills.
Ability to maintain appropriate professional boundaries and confidentiality.
Ability to work independently while functioning effectively as part of a multidisciplinary team.
Basic computer skills and experience with electronic documentation.
Must meet all applicable background screening and employment requirements.

Education

A high school diploma or equivalent is required.

An associate's or bachelor's degree in social work, psychology, counseling, behavioral health, human services, criminal justice, or a related field is preferred. Relevant professional experience may also be considered.
