Registered Nurse (RN) – Home Health

West Cleveland Area | Flexible Compensation Options

Choose the compensation package that works best for you:

$80,000 salary + full family benefits
Up to $115,000 salary with no benefits

Join our growing home health team and enjoy the opportunity to provide one-on-one patient care while working independently in the West Cleveland community.

We are looking for a compassionate, dependable Registered Nurse (RN) who wants more autonomy, meaningful patient relationships, and an alternative to the traditional hospital environment.

This position is ideal for an RN who enjoys managing their own day, developing relationships with patients and families, and providing high-quality care in the comfort of the patient's home.

Responsibilities

As a Home Health RN, you will:

Perform comprehensive patient assessments and develop individualized plans of care.
Provide skilled nursing services in patients' homes.
Monitor vital signs, medications, symptoms, wounds, and overall clinical condition.
Provide wound care, medication management, injections, catheter care, and other skilled nursing services as ordered.
Educate patients and family members regarding medications, disease management, safety, nutrition, and care needs.
Coordinate care with physicians, therapists, home health aides, and other members of the interdisciplinary team.
Communicate changes in patient condition to physicians and appropriate clinical team members.
Complete accurate and timely clinical documentation using the company's electronic medical record system.
Participate in care planning and help patients safely remain at home whenever possible.
Maintain compliance with Medicare, HIPAA, agency policies, and applicable state and federal regulations.

Qualifications

Current and unrestricted Ohio Registered Nurse (RN) license.
Current BLS/CPR certification.
Previous home health experience is preferred but not required for the right RN candidate.
Experience in hospitals, skilled nursing, rehabilitation, assisted living, hospice, or other clinical environments is welcomed.
Strong assessment and critical-thinking skills.
Excellent communication and patient-education skills.
Comfortable working independently in patients' homes.
Reliable transportation and ability to travel within the assigned West Cleveland territory.
Strong organizational and time-management skills.
Comfortable using electronic medical record systems and mobile technology.
