Full Job Description

Overview

We are seeking a dedicated and experienced Hospice Patient Care Services Designee to lead and coordinate patient care within our hospice program. This candidate will also assume the Administrator Designee role. This vital role involves overseeing clinical operations, managing interdisciplinary care teams, and ensuring compliance with healthcare regulations. The ideal candidate will possess a strong background in nursing leadership, long-term care, and hospice services, demonstrating a passion for compassionate patient-centered care. As the primary point of contact for patient care management, you will drive quality outcomes, foster team collaboration, and uphold the highest standards of clinical excellence.

Duties

Lead the development, implementation, and review of individualized care plans tailored to each patient's needs, ensuring alignment with hospice standards and regulations.
Supervise nursing staff and interdisciplinary team members, providing guidance, mentorship, and performance evaluations to promote high-quality care delivery.
Oversee documentation review processes, including medical records, care plans, and compliance reports, ensuring accuracy and adherence to HIPAA and long-term care regulations.
Manage shift assignments and coordinate staffing schedules to optimize coverage while maintaining efficient operations across multiple care settings such as assisted living facilities, nursing homes, and home health environments.
Monitor clinical outcomes through EMR (Electronic Medical Records) systems and EHR (Electronic Health Records), analyzing data to improve patient care quality and operational efficiency.
Ensure compliance with Medicare requirements and other regulatory standards governing hospice and long-term care services.
Collaborate with physicians, social workers, chaplains, and other healthcare professionals to deliver holistic end-of-life care that respects the resident’s <PERSON>’s wishes and dignity.

Qualifications

Proven experience in assisted living facilities, nursing homes, home health agencies, or hospice care environments with demonstrated leadership capabilities.
Strong background in nursing administration with supervising experience in clinical settings; management skills are essential.
BSN with 3 years of Hospice/HH experience in the last 5 years
ADN with 4 years experience within the last 5 years in Hospice/HH.
1 year of Supervisory experience in Healthcare.
Knowledge of Medicare Hospice requirements including Federal and State regulations.
Familiarity with Hospice MD systems for documentation review and clinical management.
Excellent understanding of HIPAA compliance requirements related to medical records and patient confidentiality.
Demonstrated ability to manage care facility teams effectively while providing administrative clinical supervision.
Prior nurse management or nursing leadership experience within Medicare-certified programs preferred.
