We are seeking a Registered Nurse Case Manager to plan, coordinate and provide nursing care for patients. This includes identifying their needs and arranging all necessary care measures. You will also assess the patient’s condition, including physical and emotional health, and determine the best treatments. You should have a high level of initiative and be good at interpersonal communication to ensure smooth processes and build positive relationships with patients. The goal is to ensure that our clients receive the best possible care. The role will involve direct patient care as well as administrative duties like scheduling appointments, updating medical records and maintaining accurate records of supplies and medications.

Responsibilities:

Performing nursing duties as needed including patient follow-up, visits, death

pronouncements

Supervise LPN and work as a team leader
Work closely with the Catalina hospice leadership team in an effort to deliver high-

quality hospice care to each of our patients

Educate patients about health maintenance and disease prevention
Document findings and Catalina hospice EMR within 24 hours of patient encounter
Provide advice and emotional support to patients and their family membersQualifications:
Registered Nurse with a valid license in the state of Arizona or Compact Nursing State.
Proof of car insurance
Bilingual in Spanish preferred but not necessary
Ability to build rapport with patients
Strong problem solving and critical thinking skills
Ability to thrive in a fast-paced environment

Job Types: Full-time, Part-time

Pay: $80,000.00 - $95,000.00 per year
