Intake/Referral Coordinator

Salary: $24.00 per hour

What You Will Do:

Serve as the first point of contact for prospective clients, referral sources, and caregivers, providing professional and compassionate customer service.
Coordinate the intake process from referral to admission, ensuring all required documentation is complete, accurate, and compliant with agency policies.
Communicate with physicians, referral sources, patients, families, and internal staff to facilitate timely initiation of services.
Maintain accurate client records and enter information into agency systems while ensuring confidentiality and HIPAA compliance.
Answer incoming phone calls, respond to inquiries, and direct calls appropriately.
Assist with interviewing caregiver applicants and support recruiting efforts as needed.
Perform a variety of administrative and office support duties to ensure efficient daily operations.
Demonstrate a willingness to learn all aspects of the home care agency, providing cross-coverage and support for various office functions as needed.
Collaborate with the leadership team to identify opportunities for process improvement and contribute to the continued growth of the agency.
Participate in community outreach and networking activities to cultivate new referral partnerships and maintain existing professional relationships.
Perform other office duties and special projects as assigned.

What You Will Have:

High school diploma or equivalent (associate's degree or healthcare-related experience preferred).
Previous experience in healthcare, home care, medical office, customer service, or intake coordination preferred.
Excellent communication and interpersonal skills with a professional and friendly demeanor.
Strong organizational skills with the ability to prioritize multiple tasks in a fast-paced environment.
Proficiency with Microsoft Office and the ability to learn agency software systems.
Strong attention to detail and commitment to maintaining accurate records.
Ability to work independently while also collaborating effectively with a team.
A positive attitude, willingness to learn, and flexibility to assist with various office responsibilities.
