Job Description:

$65,000 - $75,000 annual salary, based on experience

Key Responsibilities:

Supervise, coach, and support staff responsible for home visits, tele-health supervisory visits, client support, and family communication.
Oversee staff schedules, caseload priorities, documentation standards, and timely follow-up with clients, caregivers, and families.
Monitor the quality and consistency of home visits and tele-health visits to ensure families receive professional, person-centered support.
Serve as an escalation point for complex client, caregiver, and family questions related to CES, SLS, IHSS, Community First Choice, and related Medicaid services.
Conduct home visits / tele-health visits as needed.
Train and mentor staff on company procedures, client service expectations, documentation requirements, and effective family communication.
Review staff performance, identify training needs, and help team members improve accuracy, responsiveness, and service quality.
Partner with leadership to improve workflows, strengthen compliance practices, and support growth across assigned service areas.
Assist with client/family inquiries, special projects, and operational priorities as needed.

Qualifications:

Bachelor's degree required; degree in human services, social work, healthcare administration, business, or a related field preferred.
3+ years of professional experience in client services, healthcare, Medicaid waiver programs, case management, home care, or a related field.
1+ year of supervisory, team lead, training, or staff management experience preferred.
Knowledge of IHSS, CES, SLS, Community First Choice, or Colorado Medicaid waiver/program requirements strongly preferred.
Strong communication, organization, problem-solving, and follow-through skills.
Ability to support families with empathy, professionalism, and attention to detail.
Comfort working in a hybrid environment and using technology to manage documentation, communication, and staff coordination.
