JOB DESCRIPTION

Position: Qualified Professional

Division: Clinical Management Team

Reports to: Clinical Supervisor

Supervises: Associate Professionals, Program Directors and/or Team Leaders

POSITION SUMMARY: Individual responsible for monitoring the clinical operations of habilitation facilities and applicable staff, and for promoting personal growth and independence of service recipients in accordance with state and program regulations, WesCare policies and procedures, and applicable accreditation standards.

POSITION REQUIREMENTS:

1. Education: Bachelor Degree in Human Services-related field or licensure as a Registered Nurse, post bachelor’s degree with a minimum of two years of full time accumulated MH/DD/SA experience with the population served, or a Bachelor’s Degree in a non-human services field with four years of full time, post-bachelor’s accumulated MH/DD/SA experience with the population served.

3. Good knowledge of habilitation scope of services, state-specific regulations, and applicable accreditation standards.

4. Good interpersonal skills.

5. Reliable transportation.

6. Valid state driver’s license and current automobile insurance.

7. Current state medication administration certification, or attendance certificate from state medication administration course if an RN.

8. Criminal background check, abuse registry and sex-offender registry check.

POSITION RESPONSIBILITIES:

GENERAL RESPONSIBILITIES:

1. Demonstrate support of all outcomes for service recipients supported. (Identity, Autonomy, Affiliation, Attainment, Safeguards, Rights, and Health and Wellness.)

2. Insure personal growth and independence of service recipients through initial development and ongoing support, monitoring, supervision, and training of staff.

3. Listen and respond to the needs of service recipients, parent/guardians, area program staff, and agency board members.

4. Develop and maintain relationships with area program staff and families.

5. Know, understand, and abide by agency “Mission and Values” and quality of service standards.

6. Assure quality and timelines of all documentation in accordance with all state and agency requirements.

7. Attend all mandated training and maintain all current state and agency requirements.

8. Communicate in a professional manner with service recipients, staff, employers, parents/guardians, area program staff, and agency board members.

9. Assist staff in developing their ongoing job skills and supervise the provision of clients’ services.

10. Advocate for service recipients in all situations.

11. Promote agency growth through quality services.

12. Abide by all agency policies and procedures.

13. Perform job duties in accordance with agency mission and values.

12. Monitor for and participate in the QA/QI Plan as it pertains to the program, ensuring quality in the health and safety of service recipients.

13. Other duties as assigned.

PHYSICAL/ENVIROMENTAL DEMANDS;

Must be able to lift or support approximately 75 lbs. either assisted or unassisted.

Must be able to stand for extended periods of time, up to 8 hours. Must be able to

stoop, bend, and reach overhead.
