HOUSING & OUTREACH NAVIGATOR

General Summary: The Housing & Outreach Navigator conducts street outreach to identify and support unhoused community members by connecting them to housing resources, partnering agencies, and the Coordinated Entry System. This position plays a critical role in facilitating encampment closure while ensuring every individual receives compassionate, person-centered support toward stable housing. Full-time (40 hours/week) with varying schedules based on program needs.

Specific Duties and Responsibilities: The statements describe essential functions but are not all-inclusive. Individuals may perform other duties as assigned. Percentages estimate time spent on each duty.

Street Outreach and Encampment Support: (50%)

Conducts daily outreach within assigned encampments using trauma-informed strategies
Partners with unhoused community members to build trust and facilitate program enrollment
Monitors encampments with a harm-reduction approach, prioritizing safety and well-being
Educates individuals about housing resources, support services, and program benefits
Utilizes de-escalation skills and crisis intervention techniques during high-stress situations
Assists individuals in obtaining vital documents (e.g., ID, Social Security card, birth certificate, HUD McKinney documentation)
Provides advocacy, information, and referral services throughout the engagement process
Responds to crisis situations, mediates conflicts, and contacts emergency personnel when necessary
Reports daily outcomes to supervisor and immediately communicates any incidents
Interacts effectively with diverse populations, treating each individual with respect and dignity

Housing Navigation and Assessment: (30%)

Enrolls eligible clients in Coordinated Entry
Ensures up-to-date housing assessments are entered in HMIS; completes within 48 hours when needed
Completes VI-SPDAT for clients without a current assessment in ClientTrack
Identifies suitable housing options and collaborates with clients on housing applications
Utilizes By-Name Lists for effective tracking and client prioritization
Advocates for clients throughout the housing placement process
Arranges transportation for housing appointments, viewings, and move-in processes
Facilitates a seamless handoff to the assigned Rapid Rehousing Team upon housing acceptance
Coordinates emergency temporary housing for By-Name List individuals prior to encampment closures

File Management and Data Entry: (20%)

Maintains accurate client files, case notes, and documentation in accordance with program requirements
Enters data efficiently into HMIS and other required databases within specified timeframes
Completes all required forms, including housing assessments, intake documentation, and progress reports
Ensures compliance with documentation standards and program protocols

Working Schedule:

Monday - Thursday: 8:00 AM - 4:00 PM (Encampment support/Office-based work)

Friday: 10:00 AM - 12:00 PM (All-hands staff meeting/Administrative Tasks - in person every other week)

Schedule Disclosure: Please note that on Canvassing Days and Encampment Closure Days, the regular work schedule will be adjusted to accommodate these critical program activities. You will be notified in advance of any schedule modifications required for these priority operations

Knowledge, Skills and Abilities:

Effectively communicate and collaborate with individuals experiencing homelessness and with community agencies
Knowledge of trauma-informed care principles and harm-reduction strategies
Ability to work independently while managing multiple priorities and meeting deadlines
Represent oneself and the organization professionally at all times
Respond appropriately to sensitive inquiries, complaints, and crisis situations
Strong knowledge of community resources, housing programs, and social service partnerships
Demonstrate self-motivation, high energy, strong organizational skills, and sound decision-making
Maintain strict confidentiality and uphold professional boundaries
Valid driver’s license, reliable transportation, and ability to transport clients safely
Bilingual capabilities preferred

Qualifications:

Bachelor’s degree in Social Work, Human Services, Psychology, or related field preferred; equivalent experience considered
Experience working with individuals experiencing homelessness strongly preferred
Knowledge of housing programs, Coordinated Entry systems, and HMIS data management
Strong understanding of trauma-informed care and harm-reduction principles
Excellent interpersonal and communication skills
Ability to work both independently and collaboratively
Cultural competency and sensitivity when working with diverse populations
