Full Job Description

Job Overview

We are seeking a compassionate and dedicated Home Health Aide to join our family and provide exceptional in-home care to our family member.

The patient is a female in her 30's she uses IV medication and a feeding tube.

Training provided.

Staff will work both in and out of the home.

This is a full time position.

All positions are paid roles that uphold the highest standards of safety and professionalism.

3 shift Selections available

Working hours: -10am-6pm, 12pm-8pm, or 2pm-10pm Monday-Friday /

Currently No weekends

Duties

Assist patient with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility
Monitor patient health by observing vital signs like blood pressure, pulse, respiration, and temperature
Administer medications accurately following medical instructions and maintain detailed medical documentation in Electronic Health Record (EHR) systems
Support patient with medical procedures including catheter care, wound care, and other basic nursing tasks under supervision
Provide companionship and emotional support to reduce feelings of loneliness or confusion, especially for dementia patient with mild or similar signs of dementia or Alzheimer's disease
Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment
Be willing to take patient out on community engagements, outings and activities for socialization and growth opportunities
Be open and willing to speak with the family and medical teams with positive collaboration so that concerns and treatment plans can be addressed
Follow home healthcare regulations and maintain patient confidentiality in compliance with HIPAA standards

Skills

Strong patient observation skills to identify changes in condition or behavior
Experience working in assisted living facilities, nursing homes, or home healthcare settings
Familiarity with EMR (Electronic Medical Records) systems and medical documentation procedures
Knowledge of medical terminology, medication administration, and vital signs measurement
Ability to perform patient transfer methods safely using tools like Hoyer lifts or transfer belts
Proficiency in providing dementia care, hospice support, and end-of-life assistance with compassion and professionalism
Excellent communication skills for effective client interaction and collaboration with healthcare teams
Physical stamina for heavy lifting, assisting with mobility, and performing caregiving tasks over extended periods
