Advanced Practice Provider - Continuum of Care

Schedule: Full-Time

We are seeking an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to practice across our continuum of care. The role spans three settings — urgent care, primary care, and our Redstone inpatient detoxification and stabilization program — and we will build it around the provider we hire. You might concentrate in a single setting, split your time across two, or work the full continuum: caring for a patient from detox stabilization, to acute urgent visits, to long-term primary care.

There is no single “right” mix. We are looking for an excellent provider and will shape the role around your interests, strengths, and goals — whether you want to focus in one area or grow across all of them.

Key Responsibilities

Urgent Care

Provide urgent care services to patients across the lifespan; diagnose and treat acute illnesses, infections, and injuries.
Perform setting-appropriate procedures (e.g., laceration repair, incision and drainage, splinting, wound care).
Order and interpret diagnostic studies, including laboratory, imaging, and point-of-care testing.

Primary Care

Build and grow a continuity panel — establishing recurring follow-up and developing the primary care service line at your site.
Manage chronic conditions such as hypertension, diabetes, asthma, and hyperlipidemia; deliver preventive care and annual wellness visits.
Participate in chronic care management and care-coordination programs (CCM, TCM, RPM).

Redstone — Inpatient Detox & Stabilization

New to addiction medicine? That's welcome here. Structured protocols and physician support are provided, and training is available to help you build skill and confidence in this setting.

Manage medically supervised withdrawal using standardized, evidence-based, protocol-driven regimens, guided by validated scales (COWS, CIWA).
Round on inpatients and manage co-occurring medical and psychiatric conditions through a provider-review model.
Partner with the interdisciplinary care team — including behavioral and mental health, nursing, and case management— to support each patient's recovery.
Lead discharge planning focused on each patient's lasting success in recovery and a smooth transition to ongoing care.
Maintain strict confidentiality of substance-use-disorder records under 42 CFR Part 2 and HIPAA.

Across Any Setting

Collaborate with physicians, nurses, and team members; consult and refer appropriately.
Educate patients on health maintenance, disease prevention, and treatment plans.
Maintain documentation aligned with regulatory, billing, and quality standards (Joint Commission, CMS, and system protocols).
Support quality, patient-safety, and Joint Commission survey-readiness initiatives.

Qualifications

Education: Master's or Doctoral degree from an accredited Nurse Practitioner program (MSN/DNP), or graduation from an accredited Physician Assistant program (MPAS).
Licensure: Current, unrestricted Texas RN and APRN license through the Texas Board of Nursing, or current Texas Physician Assistant license.
Certification: Current national certification — Family Nurse Practitioner (FNP) strongly preferred for across-the-lifespan care — via AANP or ANCC, or PA certification through the NCCPA.
Prescriptive authority: Texas prescriptive authority with DEA and Texas DPS controlled-substance registration, or eligibility to obtain prior to start.
Life support: Current BLS; ACLS/PALS as required by assigned setting.

Experience:

Experience: 2+ years as an Advanced Practice Provider in urgent care, primary care, family practice, or emergency medicine preferred.
Experience in substance use disorder treatment or a related field.
Proven track record of working collaboratively within a multidisciplinary team.
