Assessment and Resource Specialist

Job Type: Full-time

Salary Range: $44,000.00 - $48,000.00

Reports to: Supervisor of Direct Support

About Us: The Long Island Coalition for the Homeless (LICH) is dedicated to addressing homelessness on Long Island by collaborating with non-profits, government agencies, and community partners. As a leading advocate and service provider, we work to support individuals and families experiencing homelessness through a range of programs and initiatives. Our efforts include direct assistance, outreach, resource distribution, and emergency support. Additionally, we focus on advocacy and systemic change to expand access to vital services and housing solutions, striving to make homelessness rare, brief, and nonrecurring. Program Summary – Coordinated Entry (CE): The Coordinated Entry (CE) program serves as a centralized access point for individuals and families experiencing or at risk of homelessness on Long Island. Through standardized assessments, resource coordination, and diversion strategies, CE connects households to appropriate housing and supportive services. The program works collaboratively with community partners to ensure a consistent, efficient, and equitable response to homelessness, with the goal of helping individuals and families achieve stable housing as quickly as possible.

Position Summary: The Assessment and Resource Specialist is responsible for conducting assessments for individuals and families who are Medicaid members and/or who are experiencing homelessness on Long Island. The role will require a combination of office and fieldwork and will be expected to be highly mobile.

This position supports Social Care Network (SCN) initiatives through the screening, referral, navigation, and coordination of health-related social needs (HRSN) services for eligible Medicaid Members. The role will work within evolving regional and statewide systems designed to strengthen access to housing, health-related supports, and community-based services.

This role also involves providing additional housing-related services to eligible individuals and households and providing resources to households in need. The specific tasks will evolve over time and occur in phases, based on agency and regional needs and the requirements of funding sources.

Phases & Responsibilities

Phase 1: Screener/Assessment Specialist

During this phase, the primary responsibility of this role will be to conduct health-related social needs (HRSN) screenings and provide navigation support for eligible Medicaid Members through Social Care Network (SCN) initiatives; provide referrals and information regarding resources as appropriate; document all interactions appropriately in UniteUs; and provide initial support to other staff as they complete screenings.

Phase 2 (may overlap with Phase 1, depending on need and ability): Housing Navigation Assistance

Perform housing navigation and case management support to eligible Medicaid members who have been referred for this service. Document all interactions appropriately in UniteUs.

Phase 3: CE Assessment and Resource Support

Conduct screenings/needs assessments of homeless Long Islanders. This role involves working closely with clients experiencing homelessness to conduct housing needs assessments in places such as shelters, unsheltered, in the community or over the phone to determine their needs and connect them with appropriate services and resources through the Coordinated Entry System.

This role will also monitor and respond to helpline calls within business hours, providing immediate support, information, and referrals to individuals experiencing or at imminent risk of homelessness.

Key Roles and Responsibilities:

Conduct comprehensive assessments with eligible, targeted individuals and families using established localized assessment tool and protocols (specific population and referral mechanisms will vary by phase).
Aid with short term housing problem solving and resource connections (diversion) for people that are experiencing homelessness, including developing individualized service plans based on client needs, strengths, and preferences related to housing (phases 2 and 3).
During business hours, answer and respond to helpline calls within 48 hours of message being left from individuals experiencing or at risk of homelessness (phase 3).
Provide information and referrals to callers and households being assessed, ensuring they are connected to the most appropriate services.
Uphold and ensure privacy and confidentiality for all clients and persons experiencing or at risk of homelessness.
Track call volume and trends, prepare and submit timely reports on assessment outcomes, helpline activity, and client progress.
Triage calls and referrals as appropriate to crisis, health, outreach, and case management supports.
Participate in CE Team meetings, All Agency Meetings, and other meetings relevant to job scope as approved by supervisor and work collaboratively with a multidisciplinary team, including outreach workers, case managers, and housing providers, to ensure a coordinated response to homelessness.
Input and maintain accurate and up-to-date client information in the Homeless Management Information System (HMIS) and/or UniteUs (system will vary by phase).
Collaborate with other service providers and agencies to facilitate access to shelter, housing, and other supportive services.

Key Roles and Responsibilities include the following. Other tasks may be necessary to enhance regional strategies and the local response to homelessness.

Qualifications, Strengths, and Skills: Experience with Social Care Networks (SCN), health-related social needs (HRSN) screening/navigation, Medicaid service coordination, and/or related community-based care systems is required. Experience conducting screenings, referrals, navigation, or documentation through platforms such as UniteUs is highly desirable. Must have strong knowledge of community resources, housing options, and navigating systems serving individuals and families experiencing homelessness. Must have a commitment to the organization’s mission and goals. Must embrace a person-centered and trauma-informed approach.

Have strong knowledge of community resources, connections, housing options, and navigating the homeless system. Must be able to work effectively with diverse people and personalities and promote inclusive environments and spaces. Must be able to work as part of a team.

Should possess strong organizational and time management skills, effective verbal and written communication skills, and the ability to remain calm and focused under pressure. Must have the ability to use or learn to use computer database applications. A valid driver’s license and ability to operate an agency vehicle are required. Work hours may vary based on program and client needs.
