Bloomfield Homecare LLC

Location: Columbia, MD (office) + client homes in Howard & Anne Arundel Counties

Job Type: Per Diem / PRN — W-2 or 1099 (candidate's election)

Schedule: Standing Saturday block, 10:00 AM – 1:00 PM (caregiver training & competency) + separate, as-needed client home visits

Compensation: $45 – $57/hour (training & office hours) + $140 flat per client home assessment visit Reports to: Administrator

Job Summary

We are seeking a dedicated and energetic Registered Nurse (RN) to join our team on a per diem basis, specializing in caregiver training and in-home client assessments. This vital role involves providing comprehensive evaluations of clients' health needs, educating caregivers on best practices, and ensuring high-quality, personalized care in the comfort of clients’ homes. The ideal candidate will bring a passion for patient-centered care, excellent communication skills, and a proactive approach to supporting diverse populations, including individuals with disabilities and those requiring specialized medical attention. This position offers flexibility, meaningful work, and the opportunity to make a positive impact on clients’ lives through expert assessment and education.

Responsibilities

Conduct thorough in-home assessments to evaluate clients’ health status, mobility, safety needs, and overall well-being using standardized tools and clinical judgment.
Develop personalized care plans based on assessment findings, aligning with best practices and regulatory standards such as HIPAA and NCQA standards.
Provide caregiver training sessions focused on medication administration, infection control, remote patient monitoring, and other essential skills.
Educate family members and caregivers on managing chronic conditions such as dementia, memory care needs, and behavioral health strategies.
Perform medical documentation accurately within electronic health record (EHR) systems like Axiscare; ensure compliance with medical records management protocols.
Conduct hands-on skills demonstrations and return-demonstration evaluations against Bloomfield's competency checklists (transfers and body mechanics, bathing and hygiene, toileting and incontinence care, feeding and safe swallowing, vital signs, mobility and fall prevention, infection control).
For medication-related tasks, teach the delegated nursing task or verify the individual's competency to perform it, then document that verification.
Maintain ongoing communication with clients and caregivers to monitor progress and adjust care strategies as needed while adhering to infection control procedures.

Experience

Valid RN licensure with current state registration; BLS certification required.
Prior experience working in home care settings or community-based environments preferred.
Demonstrated expertise in patient assessment techniques across diverse populations including geriatrics, pediatrics, or individuals with developmental disabilities.
Familiarity with EMR/EHR systems such as Axiscare or WellSky; experience documenting complex medical procedures including tube feeding, ventilator management, or telemetry monitoring.
Exposure to specialty areas such as infectious disease care, occupational health protocols, or emergency medicine enhances your ability to deliver comprehensive assessments.

Preferred

Prior experience as a delegating nurse, RN case manager, or clinical supervisor in an RSA, assisted living program, or DDA setting
Experience teaching or evaluating CNAs, GNAs, HHAs, or medication technicians
Completion of the Maryland Board of Nursing delegating nurse training course
Experience with private-pay home care clients and family caregivers
