Full Job Description

Under the direction of the Administrator, the Social Worker provides leadership and direction in the efficient and effective operations of the Department of Social Work. She/he initiates, develops, administers, monitors, and evaluates the provision of all Social Work services and programs in addition to supervising the staff of the Department of Social Work in the performance of their responsibilities.

Job Functions:

Every effort has been made to make this job description as complete as possible. However, it in no way states or implies that these are the only duties that social worker will be required to perform. DUTIES/RESPONSIBILITIES:

1. Establishes and implements policies and procedures of the department of Social

Work based upon regulatory agency requirements and institutional goals and

objectives. Ensures ongoing compliance with all appropriate policies and

procedures.

2. Ensures appropriate staffing levels within the Department of Social Work through the recruitment and retention of qualified professional staff.

3. Provides ongoing in-service education and continuing education of all department staff.

4. Ensures appropriate maintenance and monitoring of the allocated budget, with a focus on being cost effective.

5. Ensures the appropriate collection of data and submits formal and informal

reports of the data to the administrator on a routine basis.

6. Actively participates on institution wide committees as assigned by the administrator.

7. Maintains a liaison relationship with all appropriate community resources to include hospitals, health centers, Social Work agencies, etc.

8. Ensures the implementation of the Residents’ Bill of Rights and ensures that all facility staff and residents are aware of these rights.

9. Actively participates in interdisciplinary team conferences.

10. Performs other duties and special projects as assigned by the administrator.

11. Through ongoing monitoring and evaluation, ensures the provision of appropriate and quality Social Work services.

12. Maintains professional affiliations and enhances professional growth and development through participation in continuing education activities.

13. Reviews standards related to programs and assists the administrator in developing written policies and procedures to insure nursing home conformity to standards of

high quality and regulatory mandates.

14. Assists all departments in understanding the type of written documentation required for compliance with standards.

15. Acts as the Discharge Planning Coordinator for the facility and evaluates

obstacles to effective and timely discharge planning. Participates in the problem solving process inside the nursing home and with community agencies and personnel.

16. Develops monthly, quarterly and annual reports, or as necessary.

17. Develops reports related to quality of care or problems which may be related to Department of Social Work treatment.

18. Develops surveys and audits related to utilization of the department’s staff and their utilization.

19. May be assigned specific program areas in the Department of Social Work, e.g., Quality assurance, staff and/or student development or special projects.

20. Assumes responsibility for contribution to resource files, including distribution of information for residents and family members to appropriate personnel.

21. May be delegated to represent the Department of Social Work of the facility in program related community meetings and conferences.

22. Performs other activities as shall be deemed appropriate in assuring services to residents and the functioning of the Department of Social Work program.

23. Participates in recruitment, selection and assignment of Department of Social Work staff Ensures efficient and integrated services of staff and coordinates its contribution

to the goals of the department and the facility.

25. Analyzes and evaluates the quantitative and qualitative performance of

department staff and recommend promotion or discharge.

26. Furthers the knowledge and skill of the Department of Social Work staff to improve its services to residents and assists it in developing its potential capacities for supervision, teaching and other administrative responsibilities.

27. Participates in planning the educational and training programs of the facility and participates in direct teaching of professional and auxiliary personnel.

28. Participates in professional development of staff and continued professional growth through formal and informal study and experience within and without the facility.

29. Participates in, initiates or develops research projects, surveys and studies.

30. Gives consultation to individuals within the facility and community regarding the personal and social needs of residents and their appropriate social treatment.

31. Provides Social Work services to residents and their families.

21. Facilitates needed communication with other disciplines on behalf of the

residents, including medical, nursing, dietary, rehabilitation and psychiatric

services.

33. Coordinates arrangements for residents and families to meet with the department of health surveillance staff as necessary.

34. Coordinates and facilitates the referral of residents for needed and requested services and outside resources not available in the facility.

35. Takes leadership in the development and expansion of social and health programs in the community.

WORKING CONDITIONS:

1. Works in office area(s) as well as throughout the facility, i.e., nurses’ stations,

resident rooms, therapy areas, etc.

2. Sits, stands bends and moves intermittently during working hours.

3. Is subject to frequent interruptions.

4. Is involved with residents, families, facility staff, visitors, government

agencies/personnel, etc., under all conditions and circumstances.

5. Is subject to hostile and emotionally upset residents, family members, personnel

and visitors.

6. Communicates with Administration, the medical staff, nursing personnel, and other department supervisors.

7. Works beyond normal working hours and on weekends and holidays when

necessary.

8. Is subject to falls, burns from equipment, odors, etc., throughout the day.

9. Is subject to exposure to infectious waste, diseases, conditions, etc., including the

AIDS and Hepatitis B viruses.

10. Maintains a liaison with the residents, their families, support departments, etc. to

adequately plan for the residents’ care.

PHYSICAL AND SENSORY REQUIREMENTS:

1. Must be able to move intermittently throughout the day.

2. Must be able to read, write, speak and understand the English language.

3. Must be able to cope with the mental and emotional stress of the position.

4. Must be able to see and hear or use prosthetics that will enable senses to function

adequately to assure that the requirements of this position can be fully met.

5. Must function independently, and have flexibility, personal integrity, and the

ability to work effectively with residents, families, facility staff and support

agencies.

6. Must be in good health and demonstrate emotional stability.

7. Must be able to relate to and work with the ill, disabled, elderly, emotionally

upset, and at times, hostile people within the facility.

8. Must be able to assist in the evacuation of residents

Requirements:

Bachelor's or Master’s Degree in Social Work (BSW or MSW) required.
Licensed Social Worker (LSW or higher) in [State] preferred.
Prior experience in long-term care or healthcare setting strongly preferred.
Strong communication, organizational, and interpersonal skills.
Compassionate, patient, and professional demeanor.
Long Term Care experience prefered.
