Director ACT Program

JOB SUMMARY: The Director for the Assertive Community Treatment (ACT) team is responsible for direct client services as a member of the clinical staff, clinical supervision of staff, and administration and leadership of the team.

DESCRIPTION OF DUTIES AND RESPONSIBILITIES:

Provide administrative oversight ensuring that all documentation, billing and clinical care meets the New York State (NYS) Office of Mental Health (OMH) and IDCC requirements.
Ensure all admissions, transfers, and discharges are in accordance with the regulatory guidelines of OMH.
Directly coordinate with the Single Point of Access (SPOA) for all referrals.
Maintain fiscal viability of the program through Medicaid billing and budgeting of program funds
Maintain and ensure proper documentation in all electronic health records (CAIRS, Maven Mental Health Portal, NIMRS, EHR)
Work with the recruiting specialist in overseeing the hiring process for the team including conducting interviews and making hiring decisions.
Provide clinical oversight to the team, including person-centered treatment planning, evidence-based practices, training and the identification of high-risk participants
Monitor the team’s overall fidelity to the ACT Model
Participate in daily team meetings to review and plan participant services
Facilitate morning meetings, case planning and assignment of contact visits.
Follow up on referrals, complete intakes, maintain census of the program
Ensure all clinical documentation is completed in a thorough and timely manner
Provide training, clinical supervision, evaluation, and professional development to all staff
Provide 24-hour crisis intervention and on-call services
Ensure that all reportable incidents are documented in NIMRS, and reported to the Justice Center in accordance to the regulatory guidelines of 4 NYCRR Part 524
Participate in monthly Incident Review Committee meetings to discuss incidents, receive feedback and consolation with agency leadership, and report back on implementation efforts
Develop relationships with key stakeholders, and community resources such as DHS Shelters, psychiatric hospitals, Coalition for the Homeless, AOT Monitors, Justice system (courts, parole, probation, etc.), SPOA
Other duties as assigned based on the needs of programming.

REQUIREMENTS

Ability to work under stress and handle crises
Demonstrates knowledge of, and supports, the ACT team mission, vision and value statements, standards and the code of ethical behavior.
Understanding of and implementation of OMH ACT regulations.

WORK EXPERIENCES:

MINIMUM: One (1) year supervisory/administration experience required
MINIMUM: One (1) year Assertive Community Treatment (ACT) experience/multi-disciplinary mobile team experience strongly preferred
MINIMUM: One (1) year specialized addiction training needed to understand and treat the complexities of addiction.
MINIMUM: One (1) year experience working with individuals diagnosed with Serious Mental Illness (SMI)

EDUCATION:

MINIMUM: Master's degree in social work, psychology, counseling or related field required.

LICENSES:

Licensed Clinical Social Worker (LCSW) or Licensed Mental Health Counselor with diagnostic privilege (LMHC-D) in the state of New York. (preferred)
Licensed Master Social Worker, Licensed Mental Health Counselor (LMSW) or Licensed Mental Health Counselor (LMHD) or Licensed Marriage and Family Therapist (LMFT) (required)
Advance CASAC (preferred)

LOCATION: Onsite

Brooklyn, New York
Travel to community locations for in-person services as needed.

SALARY: $105,000 to $115,000 annually
