Full Job Description

Required Credential:

About the Role

The CORE CPST Provider delivers mobile, community-based behavioral health interventions to adults enrolled in Medicaid Health and Recovery Plans (HARP). CPST is a goal-directed, solution-focused service designed to improve functioning, promote recovery, and reduce barriers related to mental illness and co-occurring conditions. This role requires strong clinical judgment, documentation accuracy, person-centered planning skills, and comfort working in home and community environments.

Key Responsibilities

Provide CPST services to adults with serious mental illness (SMI)
Conduct comprehensive behavioral health assessments
Develop and implement Individual Service Plans (ISPs)

Deliver evidence-based clinical interventions, including:

Individual psychotherapy and counseling
Motivational Interviewing (MI)
Cognitive Behavioral Therapy (CBT)-informed strategies
Harm reduction approaches

Support individuals with:

Symptom stabilization
Crisis prevention planning
Community integration
Independent living skill development

Coordinate care with other client providers including:

Health Home Care Managers
Psychiatric providers Primary care providers
Community-based service providers
Maintain accurate, Medicaid-compliant documentation
Participate in clinical supervision and interdisciplinary team meetings

Qualifications

Candidates must possess one of the following New York State Licenses or permits:

Licensed Master Social Worker (LMSW)
Licensed Clinical Social Worker (LCSW)
Licensed Mental Health Counselor (LMHC)
Mental Health Counselor Limited Permit
Licensed Marriage and Family Therapist (LMFT)
Marriage and Family Therapist Limited Permit
Licensed Creative Arts Therapist (LCAT)
Creative Arts Therapist Limited Permit
Licensed Psychologist
Psychology Limited Permit

Additional Requirements:

Experience working with adults with serious mental illness (SMI) and/or co-occurring substance use disorders.
Strong clinical documentation and treatment planning skills.
Knowledge of Medicaid-funded behavioral health services.
Strong crisis intervention and risk assessment skills.
Ability to work independently in community settings while maintaining professional boundaries.
Strong communication, engagement, and documentation skills.
Willingness to work flexible hours, including evenings or weekends as needed.

Schedule and Work Environment

Flexible scheduling
Day, evening, and weekend availability as needed
Services are delivered primarily in community-based settings, including members’ homes and other appropriate locations in the community.
This position requires local travel to meet individuals where services are most effective.

We are currently hiring in all five boroughs of New York City – Bronx, Brooklyn, Manhattan, Queens, and Staten Island.

Pay: $80.00 per hour
