Family Empowerment Coach

Reports To: Executive Director
Salary: $48,000–$55,000 annually
Employment Status: Full-Time | Exempt

The Family Empowerment Coach provides individualized coaching, case management, advocacy, and resource coordination to children, youth, and families. Guided by a whole-family (Two-Generation) approach, this position partners with families to build on their strengths, reduce barriers, and promote long-term stability and self-sufficiency. Working collaboratively with schools, community partners, and internal programs, the Family Empowerment Coach helps families achieve meaningful outcomes while ensuring quality service delivery and compliance with program and grant requirements.

Position Summary

The Family Empowerment Coach provides individualized coaching, case management, advocacy, and resource coordination to children, youth, and families. Working collaboratively with schools, community partners, and internal programs, this position helps families overcome barriers, strengthen protective factors, and achieve sustainable outcomes through relationship-based support and effective case management.

Key Responsibilities

Provide comprehensive case management services to assigned families.
Conduct assessments, develop individualized service plans, and monitor progress toward established goals.
Conduct home visits and maintain consistent communication with families.
Connect families with community resources and supportive services that promote long-term stability.
Collaborate with schools, community agencies, and service providers to coordinate care.
Advocate for families while promoting self-sufficiency and independence.
Maintain timely case notes, client records, and required documentation.
Collect outcome data and ensure compliance with agency policies and program requirements.
Participate in team meetings, professional development, and continuous quality improvement initiatives.

Qualifications

Bachelor's degree in Social Work, Human Services, Psychology, Education, or a related field preferred.
Minimum of two (2) years of case management experience required, preferably working with children, youth, or families.
Experience conducting assessments, developing service plans, and coordinating community resources.
Knowledge of grant-funded programs, including program compliance, documentation requirements, performance measures, and reporting expectations.
Experience working within grant-funded programs and maintaining compliance with funding requirements preferred.
Strong interpersonal, communication, organizational, and problem-solving skills.
Ability to build trusting relationships with diverse families while maintaining professional boundaries.
Experience working with individuals and families experiencing economic hardship or crisis is preferred.
Proficiency in Microsoft Office and electronic case management systems.
Valid driver's license, reliable transportation, and current automobile insurance.
Successful completion of required background screening and Diocese of Belleville Child Protection Training.
