Full Job Description

Job Summary
We are seeking a dedicated and compassionate individual to join our Revere Home Care team. The Home Care Manager will oversee client care plans, coordinate home care services, and actively expand business opportunities through strategic relationships and community outreach. This hybrid role requires exceptional skills in both client-centered care management and business growth initiatives. Competitive base plus bonus compensation structure.

Responsibilities

Build and maintain strong relationships with healthcare providers, referral sources, and community partners.
Identify and pursue new business opportunities to expand the home care client base.
Develop and execute business development strategies including networking, marketing initiatives, and community engagement.
Track metrics and provide reports on care outcomes and business development progress.
Manage client communications and resolve issues promptly and professionally.
Train and support home care staff as needed to maintain care quality standards.
Assess client needs and develop individualized care plans to meet those needs effectively.
Monitor client progress and adjust care plans as necessary based on evolving health conditions and personal care needs.
Serve as a primary point of contact for the clients and their families, providing education, resources and support.
Facilitate communication between clients, families, and healthcare providers to enhance understanding of medical conditions and treatments.
Document all interactions and updates in the client's records accurately to ensure compliance with all home care standards.
Advocate for the clients’ needs and well-being.
Oversee recruitment, hiring, orientation, and ongoing professional development of staff.
Coordinate with the scheduler to assign and monitor caregiver shifts and coverage.
Conduct initial client assessments and periodic reassessments, and develop individualized care plans.
Ensure compliance with all local, state, and agency regulations and policies.
Perform annual performance evaluations and in-field supervision visits.
Oversee training efforts to ensure caregivers are informed and compliant.

Skills

Demonstrated success in business development, sales, or client acquisition roles.
Strong knowledge of home care services, client needs, and healthcare regulations.
Exceptional interpersonal, communication, and negotiation skills.
Ability to manage multiple priorities and work independently.
Experience working with KPIs in a performance-driven environment.
Familiarity with caregiver scheduling systems.
Ability to manage competing priorities in a fast-paced setting.
Marketing and outreach experience.
Excellent communication skills to effectively interact with clients, families, and healthcare professionals.
Ability to work collaboratively within a multidisciplinary team setting.
Strong organizational skills with attention to detail in managing client documentation and care plans.
