Position Overview
Gateway to Hope (GTH) is a Missouri-based women’s health nonprofit organization serving women across Missouri and Southern Illinois. GTH is growing our patient navigation team and seeks an experienced and professional case manager to fulfill the role of Patient Navigator for patients living with breast cancer. GTH has two programs serving individuals in Missouri, one of which is our Treatment Navigation Program. In that program, we currently serve hundreds of women living with breast cancer annually. This role focuses on deep and meaningful long-term case management and supportive services for patients diagnosed with Stages 0 through IV breast cancer.
Case management in this program lasts from the time of diagnosis through the end of treatment and, in some cases, the duration of the patient’s life. The person in this role will assist GTH program recipients in navigating their breast cancer treatment options, healthcare systems, healthcare payment options, and more. GTH seeks an individual who will demonstrate professionalism and compassion, as well as a deep knowledge of trauma-informed care and the unique emotional and financial needs of those battling a chronic or incurable disease. This person will also show dedication to quality work, our mission, and the communities we serve.
The Patient Navigator is a valued member of the Program Team, reporting to the Senior Clinical Program Manager. Additionally, this role will conduct outreach and education to key community stakeholders. This person will work independently and closely with the team, collaborating with GTH colleagues as needed to ensure the quality of programs and services are being delivered and evaluated regularly.

Primary duties and essential functions
Patient-Centered Care: Patient Navigation (80% of your time)
- Intake new patients: obtain necessary demographic, medical, and financial information to determine program eligibility; evaluate patient psychosocial needs using internal tools, clinical judgment, and interactions with patient and treatment team; and complete internally necessary assessments of the applicant utilizing the information provided to determine patient-centered GTH care.
- Collaborate with patients through their cancer journey: determine goals and activities designed to increase positive health outcomes and quality of life while participating in GTH programming; provide emotional support services to individuals as needed; facilitate financial assistance portion of programming, based upon policies and procedures, including bill entry in organization payment system; and empower patients to be own best advocates through varying interventions when navigating complex healthcare systems.
- Educate & engage patients: Educate patients and the community on breast health and breast cancer basics that are rooted in research and data while centering the person; assist patients with completion of advanced healthcare directives; connect patients to programming available through palliative and hospice care; and in collaboration with peers, develop and support patient-centered events, with clearly defined goals and objectives that are true to organizational values.
- Maintain exceptional data quality: track and enter accurate patient data promptly through an online database, Salesforce; engage in ongoing database training and support; and collaborate with the Senior Clinical Program Manager to better utilize instructive data to serve patients.
Patient/Public Inquires: Field Questions Related to Resources, Programs, and Services (5% of your time)
- Be a resource: Provide information to multiple stakeholder groups, including referral sources and community members, regarding GTH services and resources available to individuals seeking care; and in coordination with the Program team, respond to all inquiries, either by referring out to other resources or starting the application process for GTH services.
Outreach & Engagement: Participate in Community Efforts (5% of your time)
- Cultivate public interest in breast health: With Senior Clinical Program Manager, identify and participate in outreach opportunities to expand the reach of the GTH program and services to palliative and hospice care groups; and experience, as needed, in health fairs, activities, and events to promote GTH awareness in the community
- Be a regional leader: With the Program Team, participate in professional groups and coalitions to further the mission of GTH and late-stage breast cancer programming; maintain and develop relationships with referral sources (e.g., social workers, nurses, navigators, palliative care/hospice agencies, etc.); and collaborate with providers to ensure patients have access to additional resources.
Learning & Growth: Professional Development (10% of your time)
- Stretch into mastery: Participate in internally assigned trainings to advance clinical interventions; maintain knowledge on palliative and hospice care, especially as it pertains to breast cancer; stay up-to-date on new oncology practices and research, medications, and therapies by attending educational meetings and workshops; maintain knowledge of social security, disability, and health insurance (public and private) policies and procedures; and develop a thorough understanding of available resources at the local, state, and national level.

Characteristics and Qualifications
- Ability to recognize and articulate personal experiences relating to potentially trauma-triggering work
- Bachelor’s or Master’s Degree in Social Work preferred
- Former progress or treatment note documentation is required
- Located within and familiarity with resources in Southeast Missouri region strongly preferred
- Minimum of 3 years case management experience is required
- Proficiency in Outlook 365 is required
- Understanding of computer systems and firsthand experience working within databases required; experience with Salesforce a plus
- Excellent written, oral, and communication skills
- Detail-oriented and strong organizational skills
- Exhibiting strong interpersonal skills, a positive team player ready to interact with team members and community members regularly
- Demonstrated cultural humility is a requirement of this role, which includes a strong awareness of varying dimensions of diversity, including gender, race, and ethnicity

Compensation
Gateway to Hope provides a competitive compensation package, with an estimated salary range between $52,000-$62,000 commensurate with experience, coupled with a robust benefits package including Medical, Dental, Vision, and HSA and FSA benefits offered with applicable plans, and generous Paid Time Off, Sick, Bereavement Leave and paid Holidays. GTH paid LTD, STD, Life and AD&D insurance are included as well as an Employee Assistance Program and other lifestyle and wellness benefits. Additionally, GTH offers a 401k with Roth options as well as supplemental Life, AD&D, and Critical Illness coverage. GTH matches up to 5% of employee retirement savings contributions.
Work Environment
This position is open to anyone in the State of Missouri. If the best candidate is outside the St. Louis Metro Region, this position will be 100% remote. If the candidate is within an agreeable driving distance of the Gateway to Hope main office, this position will operate in a hybrid format. This position also regularly interacts with community members in community settings, and mileage reimbursements are provided for any agreed-upon travel. The Gateway to Hope team relies heavily on technology to achieve goals while interacting regularly with the entire staff.
Physical Demand
Some light lifting and filing are required. This role routinely uses standard office equipment and technology.
Position Type and Expected Hours of Work
This is a nonexempt position averaging 40 hours per week. Hours of work primarily between 8:00 AM and 5:00 PM, Monday through Friday, with occasional weekend and evening commitments.
