Position Summary:
The RN Case Manager is responsible for coordinating, managing, and overseeing nursing services for individuals with Intellectual and Developmental Disabilities (IDD) receiving services in home and community-based settings. The position ensures delivery of safe, person-centered, high-quality nursing services while maintaining compliance with South Carolina Board of Nursing regulations, South Carolina Department of Public Health requirements, Medicaid waiver requirements, HIPAA, and OneWell Health Care policies.
The RN Case Manager serves as the clinical liaison among individuals, families, physicians, Supports Coordinators, community providers, and interdisciplinary team members to ensure coordinated care, improved health outcomes, and regulatory compliance.
Position Scope:
The RN Case Manager manages assigned caseloads throughout the designated South Carolina service area by:
- Conducting comprehensive nursing assessments.
- Developing and updating individualized nursing care plans.
- Coordinating interdisciplinary care.
- Supervising nursing service delivery.
- Monitoring quality outcomes.
- Ensuring compliance with all applicable federal, state, payer, and organizational standards.
- Supporting referral development and staffing coordination.
- Providing clinical education to staff and caregivers.
The position exercises independent nursing judgment within the scope of the South Carolina Nurse Practice Act.
Key Responsibilities:
The RN Case Manager is responsible for coordinating and managing nursing services for individuals receiving home and community-based services. Responsibilities include conducting nursing assessments; developing, implementing, and updating individualized care plans; coordinating care with physicians, Supports Coordinators, interdisciplinary team members, and caregivers; monitoring clinical outcomes; conducting home and supervisory visits; reviewing documentation and physician orders; ensuring regulatory compliance; maintaining accurate clinical records; and participating in quality improvement and incident reporting activities.
The RN Case Manager also provides clinical guidance, mentoring, and education to nursing staff, caregivers, and individuals; supports referral and staffing coordination; participates in interdisciplinary meetings and Individual Service Plan (ISP) reviews; promotes person-centered care and preventive health practices; and performs other duties as assigned. The position provides clinical oversight but does not have direct responsibility for hiring, termination, compensation, or disciplinary actions unless specifically delegated.

Documentation Requirements:
The RN Case Manager shall accurately complete and maintain documentation including:

Certifications:
Required:
- Current South Carolina Registered Nurse License in good standing.
- Current CPR/BLS Certification.
- Valid Driver's License.
Preferred:
- Case Management Certification (CCM).
- Developmental Disabilities Nursing experience.
- Home Health Certification.
- IV Therapy Certification (if applicable).
Education:
Required:
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited nursing program.
Preferred:
- Bachelor of Science in Nursing (BSN).
- Continuing education related to IDD, case management, home health, or community nursing.
Experience:
Required:
- Minimum two (2) years of Registered Nurse experience.
Preferred:
- Home health nursing.
- Case management.
- Intellectual and Developmental Disabilities (IDD).
- Medicaid waiver services.
- Community-based nursing.

- Interdisciplinary team collaboration.

Languages:
Required: Ability to read, write, speak, and understand English proficiently. Preferred: Bilingual proficiency (Spanish or other languages commonly spoken within the service area) is preferred but not required.
Driver License:
Required: Valid South Carolina driver's license or ability to obtain one within state-required timelines. Acceptable Motor Vehicle Record (MVR). Reliable transportation. Current automobile insurance meeting state minimum requirements. Must be able to travel safely to homes, community settings, hospitals, and other provider locations.
Travel Requirement:
Frequent travel throughout assigned South Carolina service areas. Travel to participant homes, community locations, hospitals, physician offices, and agency offices. Occasional attendance at meetings, trainings, conferences, and community events. Estimated travel: 50–75%, depending on caseload and geographic assignment.
