Full Job Description

The Acute Care & Intensive Care Unit Registered Nurse (RN) renders professional nursing care to patients within an assigned unit through the nursing process of assessment, planning, implementation, and evaluation in support of medical care as directed by the medical staff and in accordance with the objectives, policies, and procedures of the hospital. The Acute Care & Intensive Care RN is responsible for providing safe, compassionate, and patient-centered care while collaborating with interdisciplinary healthcare teams to ensure optimal patient outcomes. The RN monitors patient conditions, administers medications and treatments, documents patient care accurately, and promotes a positive patient and family experience while maintaining compliance with regulatory and professional standards.

Essential Function

Assessess physical, psychological and social dimensions of patients applies knowledge of illnesses, injuries and diseases and uses available resources in assessment process.

Provides written assessment of patients on admission and subsequently in the nursing process.

Summarizes assessment data and makes a nursing diagnosis which is charted and communicated to other patient care providers.

Plans for a formal written plan of care for patients:

Initiates plan based on nursing diagnosis and applied knowledge of specific illnesses injuries, diseases, human behavior and appropriate standards of care.

Documents specific nursing orders develops short and long term goals specifying expected outcomes following intervention.

Utilizes all available resources in planning care and consults as necessary with clinical specialists, supervisors, physicians, etc.

Updates plan of care as necessary based on continuing assessments of patient.

Implement plant of care;

Implements clinical and technical aspects of care and nursing orders in accordance with established policies and procedures. Intervenes with proper technique, procedure and safety precautions to meet individual needs of patient.

Implement medical plans through passing medications and intervention with physician orders.

Documents nursing intervention, patient response, effectiveness, complications, etc.: communicate information to peers and physicians.

Evaluates - Nursing care provided: reviews and evaluates effectiveness of intervention and implementation of plan.

Determines level of accomplishment of short- and long-term goals, and expected outcomes following intervention.

Reviews and evaluates personal nursing practice as compared to standard of care and original plan of care as supported by peers and other resource personnel.

Develop discharge plan which uses all available resources.

Assesse need for patient and family instruction:

Reviews patient and family knowledge of disease, illness and readiness to accept leaving and care for patients at home.

Identifies needs for care at home, or alternatives such as out-patient treatment, extended care facility, etc.

Estabishes and maintains interpersonal relationships with patients, visitors and other hospital personnel, while assuming confidentiality of patient information.

Provides or sponsor in-services for the patient / family or significant other.

Participates in and supports research programs and activities; also participates in nursing committees and other activities which promote the growth and development of nursing and/or patient care.

Education & Experience

Bachelor’s degree in nursing required
(1+) years of clinical experience in an acute or ambulatory healthcare setting required
Demonstrated experience supporting regulatory compliance, accreditation, or survey readiness strongly preferred
Active Registered Nurse (RN) license in Illinois required and must be in good standing
Certifications: Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS), Crisis Prevention Institute (CPI) within 30 days
