Patient Care Coordinator / Medical Office Administrator

Primary Care, Concierge Medicine

Location: Bonita Springs / Fort Myers / Naples, FL
Job Type: Full-time, on-site
Schedule: Monday–Friday, with occasional schedule flexibility based on patient-care needs
Work Settings: Medical clinic, concierge medicine office, assisted living facilities, and skilled nursing facilities

About the Position

We are seeking a dependable, organized, and compassionate Patient Care Coordinator / Medical Office Administrator to join our growing primary care and concierge medicine practice.

This position plays an important role in keeping patient care organized across several settings, including our medical clinic, concierge practice, assisted living facilities, and skilled nursing facilities. The ideal candidate communicates clearly, follows through consistently, works well with others, and genuinely cares about helping patients and supporting the healthcare team.

This is not a remote position. The role requires regular on-site work and coordination among patients, families, facility staff, medical providers, pharmacies, laboratories, specialists, and other members of the care team.

Key Responsibilities

Coordinate patient appointments, follow-up visits, testing, referrals, and procedures.
Communicate professionally and compassionately with patients and their families.
Coordinate care with physicians, advanced practice providers, clinical staff, ALF/SNF nursing teams, pharmacies, laboratories, home health agencies, and specialists.
Maintain accurate patient schedules across clinic, concierge, ALF, and SNF settings.
Monitor patient-care tasks and make sure important requests are completed and followed through.
Assist with incoming calls, patient messages, appointment requests, medication-related communications, and general office inquiries.
Prepare schedules, patient lists, facility information, and other materials needed by medical providers.
Update patient demographics, insurance information, medical records, and other information in the electronic medical record.
Request, receive, organize, and upload medical records, laboratory results, hospital records, and specialist reports.
Help coordinate hospital and facility follow-up appointments and transitions of care.
Communicate scheduling changes and patient updates promptly to the appropriate team members.
Support concierge patients with timely, attentive, and personalized service.
Assist with new-patient intake, onboarding, forms, records collection, and appointment preparation.
Use the EMR, electronic scheduling systems, Google Chat, Gmail, Google Drive, Google Calendar, and other Google Workspace applications throughout the workday.
Protect patient confidentiality and follow HIPAA requirements.
Help maintain an organized, welcoming, and professional office environment.
Identify problems early, communicate concerns appropriately, and help the team find practical solutions.
Perform other administrative and patient-coordination duties as needed.

Qualifications

High school diploma or equivalent required.
Previous experience in a medical office, primary care practice, specialty practice, senior living community, ALF, SNF, home health agency, or other healthcare setting strongly preferred.
Experience with patient scheduling, medical records, referrals, or care coordination preferred.
Strong verbal and written communication skills.
Good computer and technology skills.
Ability to learn and use electronic medical records and scheduling platforms.
Familiarity with Google Workspace, including Gmail, Google Calendar, Google Drive, Google Docs, Google Sheets, and Google Chat.
Experience with Practice Fusion, Epic, Cerner or another EMR is a plus.
Understanding of patient confidentiality and HIPAA requirements.
Reliable transportation and willingness to travel locally between the clinic and assigned ALF/SNF locations.
Ability to work on-site and maintain dependable attendance.

Preferred Experience

Primary care or internal medicine.
Geriatric medicine.
Concierge or membership-based medicine.
Assisted living or skilled nursing facilities.
Hospital discharge follow-up or transitional care.
Medical referrals, authorizations, records requests, or care coordination.
Working directly with physicians, physician assistants, or nurse practitioners.
