Full Job Description

Organizational Overview: Public Guardian Services (PGS) is a professional Guardian and/or Conservator, as appointed by the Probate & Family Court, for persons with cognitive disabilities who have no one else to help them with medical, personal or financial decisions. The Care Manager (CM) is the heart and soul of this service, providing comprehensive, care coordinated fiduciary services that are informed by a supportive, professional relationship with each of the 20-30 cases that each CM carries.

Position Summary: The Care Manager (CM) provides professional, relationship-based Guardianship and Conservatorship services, supported by a Clinical Director, an Operations Manager and an Executive Director. The CM's job is to ensure that the client has adequate medical care, housing, nutrition, social interaction, and other basic ingredients of well-being in their situation, despite challenges understanding or expressing what they need. The PGS philosophy is “person-centered,” which means providing safety and guidance to the individual, while involving him or her as much as possible in each decision, and striving to preserve the dignity, autonomy and self-direction of the person as much as possible.

Through regular visits, communications with residential or medical staff, attendance at medical appointments, team meetings, and other interaction, the CM gets to know and understand the needs and preferences of the individual. This role includes regular assessment of each client, monitoring of residential settings to ensure that necessary support is being provided, good record-keeping and regular supervision. The CM also has primary responsibility to meet the reporting requirements of the Probate Court for each appointment, which includes at least annual Care Plans for guardianships and annual Accounts for conservatorships. Utilizing good judgment and communication skills, the CM will work closely with the client, other PGS staff, family members, health care providers, the court, and various social service agencies as needed.

Essential Job Functions:

Manage client caseload of up to 30 individuals, depending upon the average acuity level of assigned cases.
Visit each client at least monthly
Get to know each client and understand their needs to the extent possible (e.g. nutrition, sleep, medications, clothing, medical care, physical/mental)
Collaborate with facilities, treatment professionals and support networks in order to manage medical care and social support for all clients
Fulfill the court’s reporting requirements in a timely manner for each appointment as guardian, conservator or representative payee
Prepare affidavits that are required in order to recertify ongoing expanded authority, or to obtain new expanded authority
Coordinate with medical providers to obtain timely Medicaid Certificates, Clinician's Affidavits and Treatment plans, as needed in order to support judicial proceedings.
Monitor or initiate applications for Medicaid, Supplemental Security Income and other public benefits programs, as needed for individual clients
Make decisions with or for client in ways that maximize client’s participation and, where possible, autonomy
In collaboration with Operations Manager, secure and manage client financial resources
Keep reliable and up-to-date clinical records for each client using employer’s software
Provide after-hours coverage for emergencies on a rotating basis.
Actively participate in regularly scheduled one-on-one supervision
Effectively contribute to the organizational life of PGS through participation in team meetings, peer collaboration, program development and/or research.
Perform other duties and responsibilities as assigned.

Education/Qualifications:

Degree in Social Work or similar human services field (Masters level preferred).
Experience as court-appointed guardian or conservator.
Experience supporting disabled elders, people with ID/DD or mental health disorders, and people with no known family or social support networks.
Curiosity about people.
Energy to reach out to persons with mental illness, dementia, and physical or cognitive limitations of all kinds.
Ability to work in a stressful environment; sense of humor.
Ability to advocate for persons with disability, including engagement in respectful, constructive dialogue to effectively address complex or sensitive social needs.
Excellent written and oral communication skills.
Proficient/skilled in Microsoft Office applications.
Driver’s License and car required.
