Our mission is to ensure that every moment counts, and we are seeking a compassionate and dedicated Hospice LPN who shares our passion for providing exceptional care.

POSITION SUMMARY:
The Licensed Practical Nurse is responsible for providing exceptional care to patients under the supervision of a Registered Nurse Case Manager, in accordance with the Plan of Care.

The LPN is a member of the Interdisciplinary Team/Group (IDT/IDG).

ESSENTIAL DUTIES:

Provides continuous assessment and monitoring of the physical, emotional and spiritual needs of the patient.
Coordinates patient care with the physician and all members of the Interdisciplinary Team ( RNCM, OT, PT, MSW, ST, Dietitian or Clinical/Nursing Supervisor) through ongoing communication and appraisal of patient’s changing condition. Refers to the appropriate team member for assistance as needed.
Ensures the educational needs of the patient and family/caregiver are met to promote optimal care.
Performs complex telephone interviews and triages as appropriate.
Participates in related services, such as community education, team training, staff/board committees and orientation of employees.
Provides appropriate pain/symptom management. Evaluates patient’s response to treatments/medications.
Provide direct care to patients as prescribed in the Interdisciplinary Plan of Care to maintain the highest level of comfort and quality of life assuming primary responsibility for case management.
Attend and participate in patient care conference as needed.
Investigate and follow through on unusual orders or requests for service or information.
Participate in the agency’s on-call rotation as prescribed by the needs of the agency to provide nursing service to clints when required outside office hours.
Other duties and responsibilities as assigned.

POSITION QUALIFICATIONS:

Minimum Experience: 1-2 years of experience in hospice/palliative care, medical surgical, public health, oncology, acute care or home health.
Graduate of an accredited practical nurse or vocational nursing program, currently licensed as an LPN in the State of Colorado
Valid Colorado state driver’s license
Proof of auto insurance.
CPR certification

PHYSICAL REQUIREMENTS:

Ability to lift/push/pull minimum of 50 lbs
Lifts/positions/ and/or transfers patients
Lifts supplies/equipment
Prolonged, extensive or considerable standing/walking/sitting
Considerable reaching/stooping/bending/kneeling/crouching
Visual and auditory acuity

Job Type: Full-time

Pay: $25-30/hr
