Full Job Description

Overview
We are seeking a dedicated and compassionate Registered Nurse (RN) to join our home care team. We have been in business for over 8 years. Family owned and operated, we serve mainly veterans in and around Etowah, Brevard, and Rosman. The ideal candidate will be experienced in home care assessments, self-motivated and have a love for healthcare and enjoys interacting with the veterans and their families. Must be willing to travel to client's homes to perform in-home assessments on a monthly basis.

General Responsibilities

1) Initial assessment of newly referred clients prior to start of care.

2) Develops POC with client and/or family members.

3) Does follow-up visits to reassessment client as required by referring agency and updates POC at that time. Plan of care will change if health conditions have changed. Performance and satisfaction of caregiver is discussed at each reassessment.

4) Does supervisory visits of caregivers, as required.

5) Reports any issues with caregivers or clients to the Agency Director.

6) In-home caregiver supervisory visits (quarterly); with the client while the caregiver is in the home (are done annually).

7) Provides individual training/reviews with caregivers in the client's home, as needed

8) Acts as a support to clients, their families, and caregivers. Mediates any issues as needed.

9) Report exposures/accident reported by caregiver staff and make sure incidents are documented and then reviewed for completion and follow-up.

10) Assist Agency Director and office staff to be sure client files are completed and signed to remain in compliance.

11) Willingness to fill in as a caregiver for clients on an as needed basis.

Qualifications

1) The applicant must have a current North Carolina RN license in good standing with the NCBON.

2) Minimum of 5 years working experience as an RN.

3) Good communication and documentation skills.

4) Reliable vehicle with Valid car insurance.

5) Must be able to lift at least 25 pounds.

6) Must be able to work independently, efficiently, and within scope of practice.

7) Must be self-starter, able to make reasonable independent decisions, and easily adapts to changes in daily schedule.

8) Must be able to work around dogs, cats, and cigarette smoke (for short periods of time).

ADDITIONAL RESPONSIBILITIES: Tasks that come up and are asked to complete that are within scope of practice. Assist Agency Director as needed. This list does not cover all responsibilities and duties involved with each position:

Assess and monitor patient health status, including vital signs and medical history
Develop and implement individualized care plans based on patient needs
Educate patients and families and caregivers concerning the care plan
Maintain accurate and up-to-date patient records using AxisCare
Ensure compliance with infection control protocols and safety regulations
Supervise and oversee caregivers

Skills

Strong interpersonal skills to effectively communicate with patients, families, and team members
