Full Job Description

Make a Difference in Home Care

Home Instead Pittsford is seeking a compassionate, organized, and experienced Registered Nurse (RN) to join our growing home care team.

This is a unique opportunity to work across both sides of our business—our Companion Care services and our New York State Licensed Home Care Services Agency (LHCSA), which provides Personal Care services.

You'll combine clinical nursing with relationship-based care, supporting clients, families, Care Pros, Aides, and our office team while helping older adults remain safe and independent at home.

We're looking for an RN who enjoys working with seniors, values autonomy, and wants to be an important part of a local home care team.

What You'll Do – LHCSA / Personal Care

As part of our NYS Licensed Home Care Services Agency, you'll provide nursing oversight and support for our Personal Care clients and Aides.

Your responsibilities will include:

Conducting initial nursing assessments and ongoing reassessments for Personal Care clients
Developing, reviewing, and updating client Plans of Care
Conducting nursing supervisory visits as required
Providing clinical oversight for clients receiving Personal Care through our NYS LHCSA
Monitoring changes in client condition, safety, mobility, and care needs
Communicating with clients, families, Aides, physicians, and other healthcare providers as appropriate
Providing clinical guidance and support to Aides
Participating in Aide orientation, education, competency evaluations, and ongoing training as appropriate
Following up on incidents, falls, hospitalizations, and significant changes in condition
Reviewing clinical documentation for accuracy and completeness
Supporting quality assurance and continuous improvement activities
Helping maintain compliance with applicable New York State Department of Health LHCSA requirements
Assisting with NYS DOH surveys, audits, and regulatory readiness

What You'll Do – Companion Care

You'll also play an active role on the Home Instead Companion Care side of our business, working closely with our clients, families, Care Pros, and office team.

Responsibilities will include:

Conducting Care Consultations with prospective and new clients and their families to understand their needs, preferences, routines, and goals
Helping determine appropriate Companion Care services and developing an individualized approach to care
Conducting Quality Assurance (QA) visits with existing clients to evaluate satisfaction, quality of service, and changing care needs
Building strong, ongoing relationships with clients and their families
Identifying changes in a client's physical, cognitive, emotional, or functional condition
Communicating changes and concerns to families and the appropriate members of the Home Instead team
Helping determine when a Companion Care client's needs may require Personal Care through our LHCSA or another level of support
Providing guidance to Care Pros when client-specific questions or concerns arise
Working with our scheduling and client care teams to help ensure successful client/Care Pro relationships
Following up on client concerns and helping resolve service or care-related issues
Supporting our commitment to providing an exceptional client and family experience

What We're Looking For

Required:

Current, unrestricted New York State Registered Professional Nurse (RN) license
Graduate of an accredited nursing program
Strong clinical assessment and nursing judgment
Excellent interpersonal and communication skills
Strong documentation and organizational skills
Ability to work independently while being an active member of a team
Comfortable meeting with prospective clients and families and representing Home Instead professionally
Valid driver's license and reliable transportation
Ability to travel locally to client homes

Strongly Preferred:

Previous NYS LHCSA experience
Home care, home health, geriatric, long-term care, or community nursing experience
Experience conducting in-home assessments and developing Plans of Care
Knowledge of NYS Department of Health LHCSA requirements
Experience supervising or providing clinical oversight to home care Aides

Who Will Succeed in This Role?

We're looking for someone who wants to be more than the RN who simply "does the assessments."

This is a hands-on role that combines nursing, client relationships, quality assurance, and home care operations.

The right person is compassionate, dependable, approachable, organized, and proactive. You should be equally comfortable conducting a clinical assessment for a LHCSA client, meeting a prospective Companion Care client for a Care Consultation, performing a QA visit, coaching an Aide or Care Pro, talking with a concerned family member, and working with our leadership team.

You'll serve as an important connection between our clients, families, Aides, Care Pros, office team, healthcare providers, and leadership.

This position includes a combination of office-based responsibilities and visits to clients' homes throughout our local service area.
