Position Summary:

Children’s Supportive Care Coordinators provide care coordination services to severely emotionally disturbed children and adolescents between 5 to 18 years of age, who have an Axis I diagnosis or two chronic conditions and reside in Brooklyn, Manhattan, Bronx and Queens. The program provides in-home services including, but not limited to, intervention and educational/vocational advocacy. Care Coordinators respond to crisis by advocacy, referrals and medication monitoring. Care Coordinators assist children and adolescents obtain entitlements and benefits as well as assist the youth and family in obtaining appropriate medical and mental health services required to enhance his/her quality of life in the community.

Major Duties:

Work with recipients and providers of service need, developing an overall rehabilitation goal, writing service plans with the person that include strategies and objectives to address their identified needs.
Assess a person’s success and satisfaction in the environment of living, working and socialization.
Facilitate service delivery, including helping with individuals make and keep appointments, and escorting individuals to appointments as necessary, and assisting recipients in arranging needed mental and physical health and psychiatric rehabilitation services. Coordinate service plan providers and monitor service quality and recipient satisfaction. Advocate and assist individuals with gaining access to Welfare, health and human services, including, but not limited to Medicaid, Food Stamps, Unemployment, Educational Services, etc.
Provide services that prevent or resolve crisis in order to prevent unnecessary use of emergency rooms and/or in-patient services, and assuring crisis intervention services 24 hours a day, 7 days a week.
Assist people in learning to use fiscal resources such as Food Stamps, scholarships, etc.  Taking and advocacy stance for people and teaching them to advocate for themselves wherever possible.
Familiarize recipients with rehabilitation skills and training in areas relevant to their identified needs, wants and rehabilitation goals.
Advice and assist recipients in overcoming difficulties, particularly those related to service delivery.
Assist recipients in developing and maintaining support, including family and community. Participate in a SCM evaluation as requested.
Provide periodic progress reports as required by NYS Office of Mental Health.

Salary;

$40,000-$41,500

Qualifications:

Bachelor’s degree required.
Five or more years’ experience in a leadership role managing corporate compliance in a non-profit setting.
Strong expertise in Microsoft Office Suite.
Must have superior organizational and analytical skills, and excellent verbal and written communication skills.
Must be able to work independently and manage multiple tasks in a fast-paced environment.
Ability to work collaboratively with a variety of people, as well as an aptitude for teamwork.
Ability to use public transportation for travel to program locations throughout New York City.
Bilingual Spanish strongly preferred.
