Full Job Description

Qualifications:

A master’s or bachelor’s degree in the health and human service field is required.
One year of experience working with people with developmental disabilities, or as a service coordinator with any population.
Experience conducting community outreach and building relationships with families, community organizations, and service providers is preferred.

Responsibilities:

The Com. Hab. Supervisor is the liaison between the Service Coordinator and all outside providers in assessing appropriate support services. She/he will provide quality assurance monitoring of all outside providers and coordinate meetings to discuss individual and programmatic needs.
Ensure that all appropriate correspondence is sent to the appropriate parties promptly.
Develop and maintain partnerships with community organizations, service providers, and advocacy groups to promote available services and expand resources for individuals and families.
Ensure that all ISPs’ reviews are conducted with families and other providers. She/he should attend ISP reviews as needed.
The Com. Hab. Supervisor must ensure that the Service Coordinators have all the needed ISP attachments (Res-hap plans, Day-hap plans, plan of protective oversights if in residential).
The Com. Hab. Supervisor should attend all appropriate OPWDD training on Community Hab. regulations and train all staff as needed.
The Com. Hab. Supervisor should meet as needed with all staff to discuss all cases and be aware of all individual-related needs and ongoing issues with the individuals, which should be appropriately monitored via supervision.
The Com. Hab. Supervisor should provide direct supervision to all staff, review and sign off on all case notes monthly.
The Com. Hab. Supervisor should conduct all inquiries for services and review all new referrals before assigning a new case to the staff. All new cases of unassigned will be maintained by a supervisor until vacant slots or new staff are hired.
The Com. Hab. Supervisor should ensure that all individual visits occur within the appropriate time for billing.
Review and complete all billable staff hours before submitting to the fiscal department. The Com. Hab. Supervisor should ensure all documented Case notes are billable, Case notes activities, and accurate in terms of the amount of time spent on each service provided each month.
Maintain all individual records for ongoing compliance.
Participate in all Case note audits and complete a Plan of Corrective Action to address all Statements of Deficiencies.
Monitor all cases when a member of staff is on vacation to ensure ongoing compliance.
Monitor all individual opinion surveys to be aware of the individual, advocate, and providers’ satisfaction of dissatisfaction.
Ensure all necessary requests for Waiver Enrollment annual updates of LOCD.
Ensure that the staff Complete Waiver Enrollment Documentation
Train all staff as needed.
Monitor all individual cases/needs on a weekly basis.
Make sure all documentation that you complete is accurate, whether it is related to services that you provide for individuals, financial or personal matters, or other documentation related to your job as a supervisor.
Read and understand the agency’s code of conduct.
Follow the agency’s code of conduct and abide by all policies, procedures, guidelines, Federal, and State laws and regulations.

COMMUNITY HABILITATION DEPARTMENT:

Monthly Billing: Review case notes and submit them to the Billing Officer.
ISP/Semi ISP.
Network with other agencies that are listed in the Brooklyn Family Support Guide.
Assist families to obtain services; schedule a Front door session, work closely with OPWDD regarding paperwork completion.
Article 16 clinic for psychosocial & psychological.
Meet with families to obtain documents: Medicaid, birth certificates, etc.
Staff professional development.
Support staff in their advocacy role.
The Community HAB Plan is developed.
