Case Manager (Part Time)

for Workforce Development & Continuing Education

Status: Part Time, In Person

Reports to Program Director

Schedule: Part-time hours up to 21 hours per week, preferably 7 hours per day 3 days per week with one-hour unpaid lunch.

Salary: $21 - $25/hour, commensurate with education and experience.

The Mosholu Montefiore Community Center (MMCC) is a vital community resource that changes the course of lives of Bronx and Manhattan residents to a degree not otherwise possible. Each year, we serve more than 40,000 preschoolers, school age children, teens, adults and senior citizens through our support, enrichment, education, and recreation programs, expanding their opportunities to live full, productive lives.

Responsibilities:

Participant Case Management: Conduct intake assessments to identify participants’ employment, educational and support service needs; Developed individualized service plans outlining career and training goals; Maintain regular contact with participants through meetings, phone calls, emails and text messages; Monitor participant attendance, progress and engagement throughout program participation; re-engage students at risk of dropping out of the program; provide crisis intervention and referrals as needed.

Supportive Services Coordination: Connect participants to community resources including housing, childcare, healthcare, and mental health services; advocate on behalf of participants to remove barriers to employment and training completion.

Employment and Career Support: Coordinate with job developers to facilitate employment opportunities

Documentation and Data Management: Maintain accurate and timely case notes and participant records in both physical files and program databases to ensure compliance with funder requirement; collect and verify participant documentation; Track participant outcomes, including enrollment, credential attainment, employment placements, and retention; maintain client confidentiality.

Collaboration and Program Support: Participate in staff meetings, case conferences and professional development activities; collaborate with instructors, employers, community partners and other program staff to support participant success; assist with program recruitment, orientations, workshops, community events and special events as needed; participate in program documentation audits both internal and external; other administrative duties as specified.

Requirements:

● Bachelor’s Degree and 2 years of relevant experience OR

● Associate degree and 3 years’ relevant experience

● Minimum of one year case management, social services, or workforce development experience

● Experience working with diverse populations including youth, unemployed adults and individuals facing employment barriers.

● Availability for some evenings and weekends

● Strong knowledge of community resources

● Strong communication, public speaking, and interpersonal skills

● Independent, detail-oriented, and deadline-driven

● Demonstrated leadership, problem-solving, and time management skills

● Team-oriented and adaptable

● Proficient in Microsoft Office and Google Suite
