CLIENT CARE COORDINATOR / OFFICE Manager – Home Care

Supervises: Caregivers, including HHA, CNA, PCA, Homemaker, and Companion Care staff
About the Position
We are seeking a professional, organized, and compassionate Client Care Coordinator to join our home care team.
The Client Care Coordinator is responsible for coordinating day-to-day client care operations, including client intake, service planning, caregiver scheduling, client communication, and ongoing service oversight.
This position serves as an important link between clients, families, caregivers, and the agency. The successful candidate will be comfortable working in both an office and client-home environment and will be able to balance client needs, staffing needs, documentation, and regulatory compliance in a fast-paced setting.
What You'll DoClient Intake & Service Coordination
- Respond to client inquiries, referrals, and intake calls.
- Complete client intake and service initiation processes.
- Conduct in-home client evaluations and follow-up visits.
- Introduce agency services to prospective clients and families.
- Participate in after-hours and weekend emergency response as assigned.
Service Planning & Oversight
- Develop initial service plans based on client assessments and identified needs.
- Coordinate service plan updates and revisions.
- Monitor services to ensure they are consistent with client needs, preferences, and care goals.
- Identify changes in client needs and communicate concerns appropriately.
- Follow up to ensure services are being delivered safely and consistently.
Scheduling & Staffing
- Schedule caregivers based on client needs, caregiver availability, qualifications, and service requirements.
- Coordinate coverage for open shifts, call-offs, and staffing changes.
- Communicate schedule changes promptly to clients and caregivers.
- Help maintain continuity of care and minimize service disruptions.
Caregiver Supervision
- Provide coaching, direction, and support to caregivers.
- Monitor caregiver performance and adherence to service plans.
- Conduct performance evaluations and provide constructive feedback.
- Address performance and service concerns promptly and professionally.
- Promote compliance with agency policies and procedures.
Documentation & Compliance
- Complete required client visit notes, service records, and other documentation accurately and timely.
- Maintain accurate and confidential client and employee information.
- Follow agency policies, procedures, regulatory requirements, and documentation standards.
- Assist with quality assurance and service monitoring activities.
Client & Family Relationships
- Maintain professional communication with clients and families.
- Respond to client concerns and work toward timely resolution.
- Monitor client satisfaction and service effectiveness.
- Conduct follow-up visits as needed.

- Maintain proficiency with scheduling systems, Microsoft Office, Google Suite, and other agency technology.
- Support administrative functions including payroll and billing.
- Talent acquisition, Caregivers training, development and retention.
- Follow all agency policies, procedures, and compliance standards
- Stay current with scheduling systems, communication tools, and software
- Handle data entry tasks with precision using Microsoft Office applications and QuickBooks accounting software
- Maintain organized filing systems, both digital and physical, ensuring confidentiality of sensitive information
- Assist with office management duties such as ordering supplies, scheduling appointments, and coordinating meetings
- Provide exceptional customer support by responding promptly to inquiries via phone or email.
What We're Looking For Required Qualifications
Education & Experience
- Associate degree or equivalent required; bachelor’s degree preferred
- Minimum 2 years of healthcare or home care experience preferred
- Prior supervisory or coordination experience preferred
- Equivalent combination of education and experience may be considered
- Valid driver's license required.
- Current automobile insurance required.
- Ability to travel to client homes and other locations as required.
- Ability to work in both office and client-home environments.
- Strong organizational and time-management skills.
- Excellent written and verbal communication skills.
- Strong critical-thinking and problem-solving abilities.
- Ability to manage multiple priorities in a fast-paced environment.

Preferred Qualifications
- At least 2 years of healthcare, home care, senior care, or related experience.
- Previous supervisory, care coordination, scheduling, or case management experience.
- Experience working with caregivers, clients, families, or healthcare professionals.
- Experience with home care scheduling or electronic documentation systems.
Physical & Work Environment
This position involves work in both an office environment and client residences. The position requires frequent communication, computer and documentation work, and occasional standing, walking, sitting, and lifting up to 50 pounds.
The position may involve exposure to illness or bodily fluids that can occur in home care environments.

- Are you comfortable conducting visits in clients home and communicating professionally with clients and family members regarding their care and services?
- Are you comfortable coordinating Caregivers schedules, including addressing call-offs, open shifts and last minutes staffing changes?
