POSITION DESCRIPTION
TITLE: Nurse Practitioner DEPARTMENT: Health Services
RESPONSIBLE TO: Director of Health Services
RESPONSIBLE FOR: N/A
JOB SUMMARY: Plans and provides health services within the accepted scope of practice for nurse practitioners or physician assistants (hereafter referred to as NP/PA) to Shenandoah Valley Westminster Canterbury residents. The NP will promote a maximum level of health and independent functioning. Services include providing primary and episodic health care including services necessary for the promotion of health, the prevention of disease, and the early detection and treatment of illness.
REQUIRED EDUCATION, EXPERIENCE, SKILLS, LICENSURE:
1. Master of Science Degree in Nursing as Geriatric, Adult, or Family Nurse Practitioner .
2. Professional certification consistent with the specialty area (Geriatric, Adult, or Family) of the NP educational program and maintains certification.
3. Current license as RN and NP in Commonwealth of Virginia.
4. Three – five years relevant experience as an NP, preferably in the settings of Internal medicine, Long Term Care or Hospice.
5. Prescriptive authority in commonwealth of Virginia and Federal DEA license to prescribe.
6. Current CPR certification.
7. Demonstrated computer literacy Microsoft Office: Outlook, Word, Excel, and PowerPoint. Proficiency with electronic medical record systems (EMRs) is also required.
8. Ability to facilitate billing process for SVWC NP visits and services.
9. Valid driver’s license with good record (as determined by SVWC). Must have four years driving experience by hire date.
SCOPE OF PRACTICE: Is detailed in the practice agreement written and signed by the NP and the Supervising Physician.
JOB REQUIREMENTS:

5. Must attend annual mandatory inservices required by the State Licensure and all other mandatory inservices and/or meetings required by other regulatory agencies and/or by SVWC.
6. Follows and supports the policies and procedures established by SVWC.
ESSENTIAL DUTIES/RESPONSIBILITIES:
1. Independently provides primary and episodic health care which includes services necessary for the promotion of health, the prevention of disease, the early detection and treatment of acute and chronic illness and emergency care. This service may require collaboration among and referral to other health professionals, including but not limited to specialty physicians, physical, occupational or speech therapists, or social services. These services may be provided in the clinic, resident apartment, Assisted Living or Health Care and include taking comprehensive health history, performing physical examinations, performing or ordering appropriate lab tests, x-ray, and other tests, prescribing, and implementing therapy according to standard of practice.
2. Participates in and facilitates admission, transfer and discharge of residents both within Westminster Canterbury and between Westminster Canterbury and community facilities to provide both the appropriate level of care and continuity of care.
3. Performs independent assessments and treatment procedures in emergency situations.
4. Assesses need for, develops educational programs and presents education on individual or group basis.
5. Knowledgeable in research: collection and processing, interpretation and dissemination of data.
6. Independently or jointly evaluates program(s), providing input for continuation or expansion of future projects.
7. Completes legible and accurate medical records in a timely manner and on a daily basis, including completing accurate and timely chart entries on client transactions, orders, test results and progress notes summarizing client care for continuity of care between visits.
8. Establish and maintain good relationships within the healthcare community.
9. Provides emergency evaluation and treatment/referral to employees injured or ill at work.
10. Works with the Director of Health Services, Administrator and other staff as appropriate to establish, maintain and implement policies for influenza, TB surveillance, Hepatitis B, COVID and other potentially infectious diseases.
11. Develops and maintains practice protocols with the Medical Director to meet the Virginia Board of Nursing requirements for Nurse Practitioners or board certification by the National Commission on Certification of Physician Assistants (NCCPA).
12. Independently performs complete and episodic physical assessments on adult and geriatric populations. Independently orders, performs, and interprets laboratory data, including diagnostic procedures. Independently orders radiological studies. Independently orders physical, occupational and speech therapy.
13. Prescribes drugs according to an approved formulary established by the Boards of Nursing and Medicare in the Commonwealth of Virginia.
14. Independently dispenses medication under the direction of the supervising physician who is authorized to dispense.
15. Independently assesses the need for and provides counseling to patients on issues such as health promotion, disease prevention strategies, medicine administration, hormonal replacement therapy, continence management. etc.
16. Independently provides wound/skin management, ostomy management and continence management services to patients.
o Wound/Skin Management: debridement, complex fistula/tube management, product selection, risk assessment, complex treatment modalities, prevention strategies, topical treatments, nutritional assessment
o Ostomy Management: stoma selection sites, pre and post op teaching, product selection, treatment of stomal/peristomal skin complications, sexual counseling, dietary counseling, and vocational counseling
o Continence Management: behavioral training, bowel training, product selection, bedside cystometrogram, pelvic muscle exercise training, complex skin care intermittent self-catheterization education
17. Understands when referral to physicians and other health care providers is indicated and makes such referrals.
18. Provides initial emergency assessment and care.
19. Fit tested regulators may be required.
20. The NP Medicare provider number will be the number used for reimbursement for clinic services provided by the NP and all clinic nursing staff. The NP is responsible for the following:
a. Meeting and maintaining professional requirements to qualify as Medicare and other insurance provider. This includes maintaining appropriate licensing, certification, collaborative and supervisory agreements with the Medical Director.
b. Providing NP services and preparing appropriate level of documentation to meet requirements for correct insurance reimbursement.

- Knowledgeable in scope of practice laws regarding professional licensure
- Has basic understanding about public and private health insurance and third-party reimbursement issues
- Knowledge of community resources related to specialty care and other health related services
- Has and maintains good clinical skills

SPEECH
- Must be able to verbally communicate in the English language directly and over the telephone and be understood.
